Post hemorrhoidectomy Anoplasty with Y-V ap, V-Y ap, C ap, U ap, House ap and Rotational S ap are used to treat surgery refractory stenosis. A 67-year-old male was admitted with the complaints of severe perianal pain during defecation for 1 week which was associated with bleeding per rectum. After evaluation, anoplasty with diamond ap was done. The use of diamond ap is discussed in this case report.

Abstract Inflammatory myofibroblastic tumor (IMFT) of the colon is a very rare entity, characterized by proliferation of myofibroblast cells admixed with inflammatory infiltrates. The entity was first described in the late 1990s, and since then less than 30 cases have been described. It frequently mimics other neoplasm of the colon, such as adenocarcinoma, lymphoma and stromal tumor, and is very difficult to clinch the diagnosis preoperatively based on the clinical and radiological findings. Here, we discuss an uncommon case of sigmoid IMFT in a 35-year old male, presenting with bleeding per rectum and managed successfully by colectomy.

Anal Fissure is one of the major causes for pain at anal region. We are presenting case report of 38 year old male patient came to Shalyatantra OPD with chief complains of pain during and after defecation, bleeding per rectum, itching since 30 days. Intervention-In this case report patient is treated by using chakramarda ghrita for the management of acute fissure in ano chakramarda ghrita was used for local application along with sitz bath. The effectiveness of Charamarda was assessed by pain bleeding itching and healing. The patient has found effective relief from pain, bleeding, itching at perianal region using chakramarda ghrita which has anti-inflammatory activity, antibacterial, antifungal scavenging activity, antiulcer, anti-proliferative activity, antioxidant activity. The outcome of this management is that Chakramarda is effective in the management of Fissure in ano. Patients who take all follow-up after treatment will have less chance of symptom reoccurrence.

Acute ischaemic colitis (AIC) is being increasingly recognised as an uncommon cause of abdominal pain associated with fresh bleeding per rectum. It is paramount to maintain a high index of suspicion and adopt appropriate management strategies to avoid complications and inappropriate interventions. In this paper, we describe a case of AIC and review literature pertinent to the management of this condition. Keywords: Ischaemic colitis, acute abdomen, management.

Abstract Background: Cystic artery stump pseudoaneurysm (CASP) is a potentially life-threating condition that can be related to multiple etiologies, especially the iatrogenic factor owing to the increased number of hepatobiliary procedures. Most patients present with haemobilia. Here we report a rare case of unusual presentation of CASP with recurrent haemobilia and hematochezia. Case presentation: A 38-year-old patient developed bile duct injury after laparoscopic cholecystectomy which was identified by magnetic resonance cholangiopancreatography (MRCP). Later, she developed haemobilia due to CASP which was then treated by trans-arterial embolization (TAE) followed by fresh bleeding per rectum one month later. The latter complication eventually required surgical ligation.Conclusions: CASP is a rare complication after cholecystectomy that can present as upper gastrointestinal bleeding or rarely as fresh bleeding per rectum. Background: Intussusception is a rare cause of bleeding per rectum in neonates. Duplication cyst as a pathological lead point for intussusception is rarer too. Case Presentation: A female neonate presented with bilious vomiting and bleeding per rectum. Ultrasonography diagnosed it as intussusception. Intraoperatively, on reduction of intussusception, a mass was found which on histopathological examination (HPE) revealed a duplication cyst. Conclusion: A high index of suspicion is required for an early diagnosis of neonatal intussusception, which is essential for preventing complications and mortality.

Endameba histolytic a of the gastrointestinal tract is common in developing countries. There are many cases where colonic amebiasis can mimic colonic carcinoma but amebiasis and coexisting carcinoma is exceedingly rare. We present a case of 43 years old male who presented with abdominal pain, generalized weakness and bleeding per rectum. Endoscopic examination showed superficial ulceration with edematous mucosa in proximal, transverse and ascending colon which was indistinguishable from ulceration and amebomas. Histopathological examination of tissue revealed the coexistence of Endameba trophozoites with adenocarcinoma of colon. The organism was demonstrated by Periodic Acid Schiff’s stain with engulfed red blood cell. Our report is to enhance the awareness that amebiasis is may not only mimic carcinoma but can also coexist with carcinoma. After extensive search of literature, our isolated case report might be under tenth of reported cases. Keywords: Amoebiasis; Endameba histolytic; Carcinoma
