A 52 years lady presented with chest pain and shortness of breath. Chest pain was initially CCS class II and gradually CCS class III. Coronary angiogram reveals triple vessel disease for which she underwent CABG (RSVG to LAD) in December 1999. On January 2003 PTCA with stent (sirolimus - 3 x 15m m) to LCX was done. She again complaints of chest pain after a short symptom less period. Recent coronary angiogram revealed TVD with left main involvement. To avoid the deleterious effects of cardiopulmonary bypass, total arterial revascularization was performed on the beating heart, using an Octopus stabilizer, in this prospective study of 803 patients without selection bias. Single-vessel disease was present in 71 (9%) patients, double-vessel disease in 204 (25%), and triple-vessel disease in 528 (66%). An ejection fraction < 30% was found in 127 (16%) cases. Angiography was carried out before discharge in 204 (25%) patients. Grafts included left and right internal mammary arteries and the radial artery. There were 2,661 grafts placed with a mean of 3.31 grafts per patient (range, 1 to 6). Operative mortality was 0.5%. There was no postoperative stroke and few incidences of renal impairment, even in patients with chronic renal failure. Blood transfusion was not required in 558 (69%) patients. Mean hospital stay was 5.6 days. Overall angiographic patency was 98.6%. Total arterial revascularization on the beating heart was found to be safe, effective, and reproducible in almost all patients, with excellent short-term patency rates and minimal morbidity.

The management of combined Coronary Artery Disease and Peripheral Vascular Disease is a challenge and brings with it numerous clinical dilemmas. The 56 year old gentleman presented to our department with significant triple vessel disease with occluded lower end of aorta just before bifurcation and occluded bilateral superficial femoral arteries. CABG with aorto-femoral and femoro popliteal bypass was done on the same setting. On 12th postoperative day he was discharged from hospital in good general condition. The first 100 words appear below: A 60-year-old male patient was admitted into the department as a diagnosed case of ischemic heart disease and his coronary angiogram reveals triple vessel disease (80-90% lesion in the mid part of left anterior descending artery; 90% lesion in the distal part of obtuse marginal and 99% stenosis in the distal part of right coronary artery). The patient was suffering from chest pain for last 3 months which aggravated by exertion and radiates along the neck and left arm. The patient was non-diabetic and non-hypertensive.
