Urinary bladder injury is a rare complication of laparoscopic abdominal surgery. The incidence is low and usually goes unidentified with confusing presentation. We report a case of such injury during laparoscopic appendectomy that was identified on 3rd post-operative day and a small 1 cm tear was found at posterior wall of bladder which and was repaired. There was also a collection anterior to bladder which was drained separately under radiological guidance later as on table cystogram did not show any leak anterior to bladder. Patient also had covid pneumonia during stay and patients stay and recovery was prolonged. Attention must be paid to port insertion under vision especially low insertion of supra pubic port should be avoided with pre op catheterization and key to diagnosis of injury is awareness of this rare entity. Perioperative visual loss (POVL) is a drastic complication that can result after ocular and more commonly non-ocular surgery, mostly reported after spine and cardiac bypass procedures. Despite the rarity of such complication, it has been reported following the abdominal surgeries. Overall, the most common cause of POVL is ischemic optic neuropathy but any pathology to the optical system from the cornea to the occipital lobe can lead to this rare phenomenon. Here, we are reporting the second case in the literature of post-operative visual loss after laparoscopic appendectomy. A young female, with no underlying disease, underwent laparoscopic appendectomy after septic shock secondary to acute appendicitis. Postoperatively, patient complained of complete blindness after extubation. Neurologic examination revealed bilateral near complete blindness, and hemodynamic ischemic stroke in the occipital cortex.

Stump appendicitis has been reported after either open or laparoscopic appendectomy. Because of the low index of suspicion owing to a previous history of appendectomy, the diagnosis is often delayed and complications can occur. We report a 30-year-old patient with Stump appendicitis, who underwent completion appendectomy laparoscopally.

Paraduodenal hernias are rare congenital internal hernias accounting for <2% of intestinal obstruction. Left paraduodenal hernias (LPDHs) into the fossa of Landzert are the more common type and result from abnormal rotation of the midgut and failure of peritoneal fusion. Sequelae of these hernias usually occur spontaneously in the 4th or 5th decade of life and are more common in males and have a significant risk of incarceration and subsequent strangulation. We describe a case of a 15-year-old female who develops a LPDH following laparoscopic appendectomy, resulting in jejunal incarceration and subsequent small intestinal obstruction. The patient discussed is from an atypical demographic, being young and female. In addition, the precipitating event prompting incarceration of the hernia appears to be the application of pneumoperitoneum, placement in the Trendelenburg position, and manipulation of small intestine for the purpose of facilitating laparoscopic appendectomy. To our knowledge, this is the first reported case of LPDH exacerbated by laparoscopic procedure.
