The paper presents a review of three clinical cases of treating patients with chronic fractures of the odontoid process of C2 vertebra using various combinations of surgical techniques. In all cases, complete decompression of the spinal cord and stabilization of the upper cervical segment of the spine were achieved. A decrease in the range of motion in the cervical spine was observed, which did not affect patient satisfaction. A gradual regression of the neurological status and increase in muscle strength were recorded. Comparative analysis of the neurological status was carried out before and after surgical treatment. When assessing the general condition of patients using the index of disability due to pain in the neck (NDI), an improvement in the quality of life was noted. These data show the opportunities and peculiarities of methods for treating patients with chronic C2 odontoid fractures.Type of publication: description of clinical cases. Evidence level: IV.

Hallux limitus is one of the most prevalent, debilitating disorders of the first metatarsophalangeal joint, and it has many proposed etiologies. This article reviews these etiologies, focusing primarily on the pes planus foot. The pes planus foot type is often associated with symptomatic hallux limitus and the accessory navicular. This article discusses this correlation, although a causal relationship has not been proven. The prevalence and classification of the accessory navicular are also discussed. Clinical cases involving symptomatic hallux limitus occurring concomitantly with an accessory navicular are reviewed, including radiographic findings, symptoms, and surgical treatment. Background. 25G or 27G three-port vitrectomy, followed by staining and removal of the internal limiting membrane with vitreous cavity air or gas tamponade, is one of the main modern vitreoretinal surgery principles for primary macular holes. However, not all patients manage to obtain a functional outcome when the anatomical outcome is achieved. Purpose. To  present clinical cases of surgical treatment of idiopathic macular holes (IMH) exhibiting a differentiated approach.Matherials and methods. This article analyzes clinical cases of four patients with primary idiopathic macular holes, differing in terms of development, hole diameter and the presence of comorbidities. Results. The results of surgical treatment were evaluated 14 days and 1 month after surgery. In all clinical cases, surgical treatment resulted in a positive anatomical outcome by blocking a retinal defect. Best corrected visual acuity increased, on average, from 0.08 ± 0.05 to 0.4 ± 0.05.Conclusion. The analysis of the presented clinical cases clearly demonstrates the need for a timely and differentiated approach to the treatment of patients with IMH, that considers etiology of the pathological process, period of its existence as well as concomitant and general somatic pathologies.

Introduction. Emphysematous pyelonephritis (EPN) is a rare form of acute suppurative pyelonephritis, which is characterized by a rapid and severe course due to the pathogenic effect of the gas-forming bacterial flora. Currently, only a few hundred clinical cases of EPN are described in the literature. Currently, there are no standards for the surgical treatment of patients with EPN, but there is a clear tendency towards a wider use of minimally invasive methods of treatment and limitation of indications for nephrectomy. A risk-based approach based on the proposed radiological classifications of EPN is considered preferable. Purpose. Тo study all literature sources available for the current period on the Internet, describing clinical cases and experience in treating patients with a diagnosis of emphysematous pyelonephritis. Materials and methods. Тhe results of a search in the scientific databases PubMed, MEDLINE, Embase, elibrary were analyzed for the queries «emphysematous pyelonephritis», «classification», «gassing bacterial flora», «diabetes mellitus», «conservative treatment», «nephrectomy», «percutaneous drainage». 754 scientific publications were found, 25 were selected for detailed analysis. Results and discussion. Today, there are no standards for surgical treatment of patients with EP, but there is a clear trend towards wider use of minimally invasive methods of treatment and limiting the indications for nephrectomy. Minimally invasive methods of treating EN allows organ-preserving treatment and reduces the risk of renal failure. The lack of clear indications for the choice of the scope of surgical treatment and the low orientation of specialists towards radiological diagnostic criteria are the main reasons for excessive organ-carrying treatment of patients with PF. A risk-mediated approach based on the proposed clinical and radiological classifications of EP is considered preferable. In controversial cases, it is possible to recommend an attempt at minimally invasive treatment in combination with adequate conservative therapy, and only if it is ineffective, resort to performing nephrectomy. Conclusion. Indications for nephrectomy should be strictly limited and revised taking into account modern approaches. Nephrectomy should be an option of choice only if conservative treatment is impossible or ineffective.

The article presents 4 clinical cases of surgical treatment of juvenile discospondilitis in dogs at the level of LVII-SI. All animals at the first stage were undergoing to long-term antibiotic therapy without positive dynamics, both clinically and with radiographic evaluation. The surgical intervention included the laparotomy with ventral access to the vertebral bodies at level LVII-SI, removal of pathologically modified tissues and stabilization by metal construction combined with bone autoplasty. The improvement of the clinical state of the animals was observed at 3…7th day after the surgery. In the remote period the result is evaluated as excellent or good by all owners.
