Single-incision laparoscopic transabdominal preperitoneal (sil-tapp) mesh in the treatment of inguinal hernia in patients over 40 years old

Tóm tắt Đặt vấn đề: Nghiên cứu về phương pháp phẫu thuật nội soi (PTNS) một lỗ qua ổ bụng đặt mảnh ghép (MG) ngoài phúc mạc (PM) điều trị thoát vị bẹn (TVB) ở người bệnh (NB) từ 40 tuổi trở lên. Phương pháp nghiên cứu: Kết quả PTNS một lỗ qua ổ bụng đặt MG ngoài PM điều trị NB TVB từ 40 tuổi trở lên bằng dụng cụ thông thường. Đối tượng và phương pháp nghiên cứu: Hồi cứu kết hợp tiến cứu, theo dõi dọc63 NB TVB nam giới từ 40 tuổi trở lên được điều trị bằng phương pháp trên. Kết quả: Tuổi 62,03±10,06, thời gian mổ 44,29±16,48 phút, biến chứng sớm11,1%, tái phát 1,6%. Kết luận: PTNS một lỗ qua ổ bụng đặt MG ngoài PM điều trị TVB ở NB từ 40 tuổi trở lên bằng dụng cụ thông thường đem lại kết quả tốt và tỷ lệ tái phát thấp. Abstract Introduction: Study of single incision laparoscopic transabdominal extraperitoneal mesh surgery to treat inguinal hernia in patients over 40 years old. Aim of this study is to evaluated the results ofsingle incision laparoscopic transabdominal extraperitoneal mesh surgery to treat inguinal hernia in patients over 40 years old with normal equipments. Material and Methods: Retrospective and prospective study, verticalsurvey63 malepatients over 40 years old with inguinal hernia were treated by the single incision laparoscopic transabdominal extraperitoneal mesh surgery. Results: Mean age 62,03±10,06, average of surgical time 44,29±16,48 minutes, early complication 11,1%, recurrence 1,6%. Conclusion: Single incision laparoscopic transabdominal extraperitoneal mesh surgery to treat inguinal hernia in patients over 40 years old with normal equipments achieved good outcomes and low recurrence. Introduction: The laparoscopic surgery is done by using a single port. You place a device through a single incision between 18mm to 50mm and 3-4 trocars can be used. Our work consists of making 3 little incisions to put the three trocars which measure 5 mm and use conventional instruments that measure 36 cm of length of laparoscopic surgery within the internal limits of the umbilicus. Objective: Describe the surgery technique and evaluate the initial results of a laparoscopic cholecystectomy option through three intraumbilical incisions. Methods: It is a retrospective, descriptive, multicentre and observational study. Since June 2014 to August 2019, 20 patients, 17 females and 3 males, were operated using this method from the age ranges of 16 to 65 years old. The average surgical time was 65 minutes. Results: In all cases the procedures were concluded by the intraumbilical approach. One of the cases had to be re-operated due to intraabdominal bleeding from the posterior branch of the cystic artery. Conclusions: The technique is reproducible; the usual triangulation of the ports is not available. Therefore, fort this technique the surgeon needs to have experience and have had advanced training in laparoscopic surgery. The cases must be selective.
