Aesthetic and Functional Rehabilitation of Multiple Grossly Decayed Anterior Teeth: A case report

Abstract Aim The purpose of this case report is to present the esthetic and functional rehabilitation of the teeth in a 20-year-old patient with amelogenesis imperfecta (AI), facial asymmetry due to functional mandibular shift, and unilateral posterior crossbite. Background AI is a group of hereditary defects of enamel unassociated with any other generalized defects. AI results in poor development or the complete absence of the enamel of the teeth caused by improper differentiation of ameloblasts. Case Description This report describes the diagnosis and treatment of a young female patient with AI and facial asymmetry using a combined orthodontic-prosthodontic approach. Initially, the posterior crossbite, mandibular shift, and facial asymmetry were treated orthodontically. Later, metal-ceramic crowns for posterior teeth and allceramic crowns for anterior teeth were fabricated for final restorations. Summary Coordinated orthodontic and prosthodontic treatment, with careful consideration of patient expectations and requests, were critical for a successful outcome and patient satisfaction. Clinical Significance The complexity of the management of patients with AI supports the suggestion that the dental profession should have appropriate methods for the rehabilitation of rare dental disorders. ABSTRACT Introduction: Treatment of maxillofacial injuries is complex and requires the establishment of a comprehensive and accurate diagnosis and correct treatment planning. Objective: The objective of this case report was to describe the re-treatment of a 27-year-old woman who was involved in a severe car accident that resulted in the loss of five anterior teeth and alveolar bone, and whose previous orthodontic and surgical treatments had been unsuccessful. Case report: In this case, the space for the missing mandibular molar was reopened to allow for rehabilitation. The positions of the mandibular incisors were improved. The right mandibular canine was moved to the mesial, allowing for correction of the Class II canine relationship on that side, and implants were placed to replace the maxillary anterior teeth. Conclusion: Anterior aesthetic and functional rehabilitation using a multidisciplinary approach was essential to improve the patient’s facial aesthetics, to obtain great improvement in function and to achieve occlusal stability after 2 years of follow-up.

The Aesthetic

All-ceramic restoration of anterior teeth is a challenging clinical situation for which a variety of all-ceramic systems are available. The aim of this study is to present a case report of the construction of two single-unit metal-free fixed prostheses in anterior teeth. A 30-year-old male patient came to the Faculty of Dentistry, Federal University of Amazonas, concerned about his teeth. Elements 11 and 12 were endodontically retreated and fiberglass intracanal retainer posts were inserted. Dental crowns were prepared using the silhouette technique. The crowns were cemented with resin cement. The patient was very satisfied with his new smile and the aesthetic rehabilitation of the anterior teeth provided by the treatment.Keywords: Aesthetics; Oral rehabilitation; Dental porcelain.

Florid cemento-osseus dysplasia (FCOD) has been described as a reactive process in which normal bone is replaced by fibrous connective tissues and cementum-like materials. Radiographically it appears as dense, lobulated masses, often occurring bilaterally with symmetric involvement. In this case report, a successful implant placement has been reported in a 62-year-old Caucasian woman with a chief complaint of mandibular partial edentulous. Radiographic images showed the bilateral radiopaque lesions in edentulous regions of mandible, and mandibular anterior teeth alike. All mandibular teeth were vital and no root resorption was detected. The findings of X-ray images were attributable to FCOD. A highly conservative step-by-step 2-stage implant surgery was performed. After 6 months the implants loaded with fixed prosthesis. 2, 4, 6, 12, and 18 months after the surgery radiographic images were taken, which revealed an optimal functional rehabilitation and complete integration of implants. This report confirms that treating the edentulous area near the FCOD lesions could be planned, if conservative step- by-step implant placement been considered. To the best of our knowledge, a case of FCOD with successful implant placement has not been reported previously. More studies in more patients are needed to confirm results of such a therapeutic modality.

Al results in poor development or complete absence of the enamel of the teeth caused by improper differentiation of the ameloblasts. Report This report describes the diagnosis and treatment of a young male patient with AI and missing molar teeth using contemporary restorative strategies. Initially, the tooth surfaces were treated with a professional cleaning along with conservative restorative treatment. Later, metal-ceramic crowns for posterior teeth and fullceramic crowns for anterior teeth were utilized for final restorations. Summary The complexity of the management of patients with AI supports the suggestion the dental profession should have appropriate methods for the rehabilitation of rare dental disorders. Keeping in mind the final result is the basis of any type of treatment, especially those in which the morphology, size and proportion of the anterior teeth will be changed. This is where a good treatment plan based on a diagnostic wax-up tested with a mock-up and approved by the patient becomes crucial. This case report aims to exemplify how to transfer the diagnostic information to the patient’s mouth and direct it not only to the restorative dentistry, but also to the surgeon when performing the crown length. Diagnostic waxing was performed by the laboratory technician, obeying anterior posterior incisal and gingival criteria and curvatures, which were transported to an aesthetic guide through mock up to mark the surgical points. Then, surgery to increase the clinical crown with bone access and after healing, direct venners in composite resin also guided by waxing. In conclusion, a workflow can be established using the wax-up / mock-up that serves as a guide for the periodontist in the approach to surgical lengthening of the crown and for the rehabilitator who uses it to produce changes in the shape of dental dimensions .
