Angioleiomyoma aka Vascular leiomyoma is a benign mesenchymal tumour originating from the smooth muscles and containing thick walled vessels. It is commonly seen in the subcutis of the lower extremities. Although a few cases of uterine angioleiomyoma have been reported, broad ligament angioleiomyoma is extremely rare. Case History: 45 year old multiparous women presented to the OPD with complains of pain and mass in abdomen since 4 months. No history of menstrual irregularities was noted. Ultrasonography revealed a homogenous solid mass in lower abdomen region. The mass was hypodense. A provisional clinical radiological diagnosis of Broad ligament fibroid was made. The patient underwent a surgery for removal of broad ligament mass with bilateral salpingo-oophorectomy and specimen was submitted for histopathological examination. Conclusion: Angioleiomyoma of broad ligament is an extremely rare diagnosis and should be diagnosed correctly with the help of IHC markers.It must be kept in mind as a differential diagnosis of abdominal mass associated with pain to be able to arrive at a correct diagnosis. Vascular Leiomyoma (VL) is a benign soft tissue tumor rarely observed in oral tissues. The aim of this paper is to present a new case of vascular leiomyoma of the lip (VLL), describing its clinical, microscopical, and immunohistochemical features, and review the literature on the tumor. A 27-year-old woman presented with a small painful lesion on lower lip mucosa. Excisional biopsy revealed a well-circumscribed, encapsulated mass formed by small and fusiform cells. Immunohistochemistry analysis revealed intense and diffuse expression of smooth muscle actin (SMA) within the tumor cells and CD34 immunoreactivity of the endothelial cells lining the vascular spaces, indicating the presence of blood vessels. A final diagnosis of VLL was established. No signs of recurrence were observed after five years of follow-up. Although VLL is a rare lesion, it must be considered in the differential diagnosis of nodular lesions on lower and upper lips.

Reported here is the case of a 55-year-old woman presenting to a podiatry clinic with a chief complaint of left heel and ankle pain, who ultimately underwent operative excision of an angioleiomyoma adjacent to the tibialis posterior artery at the level of the medial malleolus. Accompanying this case are images from three modalities through which the defining characteristics of an angioleiomyoma can be appreciated. This case advocates for the inclusion of angioleiomyoma in the preoperative differential diagnosis of a mass presenting as a pseudoaneurysm in the lower extremity, particularly among women in the fourth to sixth decades of life.
