The authors have elaborated a new method for conservative treatment of femoral head aseptic necrosis (FHAN) at early stages of the disease (prior to cartilaginous layer collapse). The method consists of performance of prolonged epidural analgesia by administration of local anesthetic through epidural catheter with dosed speed for 6 - 8 days. Drug sympathectomy results in vascular tension drop that favours the improvement of microcirculation in hip joint region. Results of that method application in 24 patients show that it enables to cup off the pain syndrome and restore patient's functional abilities directly after treatment course completion. Remission lasts for up to 12 months. Application of the proposed method enables to postpone performance of total hip arthroplasty for the period of up to 6 years.

AbstractCalcium hydroxide is a slow-acting antiseptic substance used in several forms for the last century in various fields of dentistry. Its applications in endodontics are by far the most meaningful including the treatment of root resorptions and perforations, inducing the apexification process, and most importantly as intracanal medicaments representing a crucial step in the nonsurgical management of large periapical lesions. The aim of this article is to conduct a review of the properties, antimicrobial effect, combination with adjuvant substances, biocompatibility, cytotoxicity, and adverse effects of calcium hydroxide-based dressings as an additional manoeuvre in the conservative treatment of chronic apical periodontitis and to draw attention on the importance of this extra step.

Abstract This article presents specific aspects regarding the role of endodontic medication in the conservative treatment in chronic apical periodontitis. Starting from the bacterial microflora, the etiological factor of the periapical lesions, we discuss about the modalities of identification by the means of in vivo and in vitro tests of the microorganisms present inside the root canal. The characteristics of these methods of microbiological evaluations, as well as their clinical relevance, are reviewed. The general characteristics of endodontic antimicrobial medication are presented, as well as the causes of endodontic failures in treatment in chronic apical periodontitis. It is underlined the essential importance of the rigorous biomechanical treatment, in association with endodontic lavage, which makes the antibacterial medication only have a subordinate role in the treatment of chronic apical lesions.
