BackgroundThere has been limited research on the positive aspects of physician wellness and to our knowledge there have been no validity studies on measures of resilience and grit among internal medicine (IM) residents.ObjectivesTo investigate the validity of resilience (10 items Connor-Davidson Resilience Scale (CD-RISC 10)) and grit (Short Grit Scale (GRIT-S)) scores among IM residents at a large academic centre, and assess potential associations with previously validated measures of medical knowledge, clinical performance and professionalism.MethodsWe evaluated CD-RISC 10 and GRIT-S instrument scores among IM residents at the Mayo Clinic Rochester, Minnesota between July 2017 and June 2019. We analysed dimensionality, internal consistency reliability and criterion validity in terms of relationships between resilience and grit, with standardised measures of residents’ medical knowledge (in-training examination (ITE)), clinical performance (faculty and peer evaluations and Mini-Clinical Evaluation Examination (mini-CEX)) and professionalism/dutifulness (conference attendance and evaluation completion).ResultsA total of 213 out of 253 (84.2%) survey-eligible IM residents provided both CD-RISC 10 and GRIT-S survey responses. Internal consistency reliability (Cronbach alpha) was excellent for CD-RISC 10 (0.93) and GRIT-S (0.82) overall, and for the GRIT subscales of consistency of interest (0.84) and perseverance of effort (0.71). CD-RISC 10 scores were negatively associated with ITE percentile (β=−3.4, 95% CI −6.2 to −0.5, p=0.02) and mini-CEX (β=−0.2, 95% CI −0.5 to −0.02, p=0.03). GRIT-S scores were positively associated with evaluation completion percentage (β=2.51, 95% CI 0.35 to 4.67, p=0.02) and conference attendance (β=2.70, 95% CI 0.11 to 5.29, p=0.04).ConclusionsThis study revealed favourable validity evidence for CD-RISC 10 and GRIT-S among IM residents. Residents demonstrated resilience within a competitive training environment despite less favourable test performance and grittiness that was manifested by completing tasks. This initial validity study provides a foundation for further research on resilience and grit among physicians in training.

Abstract Background Recent evidence has shown substantial disparities in the rate of advancement to full professorship among women as compared to men faculty in academic infectious diseases (ID). We sought to identify barriers to academic advancement overall and by gender among faculty physicians in this field. Methods We conducted a web-based survey of academic faculty in ID. The survey was made available to the IDWeek2019 attendees and digitally via email and social media to the IDSA membership at large from 9/18/19 – 11/8/2019. The survey assessed demographic characteristics and barriers to faculty advancement and achievement, building on prior research. Survey themes included faculty promotion track, part-time work history and a suite of questions about workplace atmosphere and policies related to career advancement. Multivariable Poisson regression models were used to evaluate the association between these factors and full professorship. Results Of 1,036 respondents, 790 were retained in the final dataset [Men: 322 (40.7%), Women: 458 (58.0%), Other: 10 (1.3%)]. 352 respondents were Instructors or Assistant Professors (38.5%), 198 were Associate Professors (25.1%) and 240 were Full Professors (30.4%). Fewer women reported that their promotion process was transparent (57.4% v. 67.6%, p=0.004) and more women Full Professors felt they had been “sponsored” compared to men at their same rank (73.3% v. 53.6%, p=0.002). In regression analyses (Table 1), gender, publications and clinical trial leadership were significantly associated with full professor rank and promotion transparency and NIH grants emerged as possible correlates of this outcome. Salary support, part-time work, women in leadership, faculty promotion track and sponsorship were not associated with this outcome. Table 1. Results of Poisson regression analysis Conclusion Sponsorship and transparency of promotion criteria differed by gender and emerged as potentially important factors associated with full professorship in academic ID. What should the post-COVID conference look like? In our attempt to answer this question, we first describe the primary functions and affordances of conferences. Our frank appraisal reveals the breadth of reasons why academics attend conferences, and how conference attendance often blends personal and professional motivations. We also elaborate some of the shortcomings of in-person conferences, spanning personal, professional, and societal concerns. Recent alternative (virtual) formats for convening scholars provide means for alleviating some of these shortcomings, but do not seem entirely up to the task of providing a fully satisfactory solution to all that conferencing can be. Moreover, we extrapolate from prior history and ongoing trends to predict that technological solutionism to conferencing is likely to unleash both positive and negative dynamics, some of which will exacerbate current ills in our profession. We then sketch out a values-based approach that can serve as a basis for reimagining academic conferences. This vision promotes a federated model of conferencing, grounded in principles of inclusion, diversity, community, and environmental stewardship.

It is commonly thought that the promotion of faculty members is affected by their research performance. The current study is unique in examining how academic faculty members perceive the harm or damage to academic appointment and promotion processes, as a direct effect of student evaluations as manifested in teaching surveys. One hundred eighty two questionnaires were collected from senior faculty members at academic institutions. Most respondents were from three institutions: Ariel University, Ben Gurion University, and the Jezreel Valley College. Qualitative and statistical research tools were utilized, with the goal of forming a model reflecting the effect of the harm to academic appointment and promotion processes, as perceived by faculty members. The research findings show that the lecturers find an association that causes harm to their promotion processes as a result of student evaluations. Assuming that students' voices and their opinion of teaching are important – the question is how should these evaluations be treated within promotion and appointment processes: what and whom do they indicate? Do they constitute a reliable managerial tool with which it is possible to work as a foundation for promotion and appointment processes – or should other tools be developed, unrelated to students' opinions?
