The association between emphysematous pyelonephritis (EPN) and diabetes mellitus is well known. EPN as the cause of sepsis in hepatocellular carcinoma (HCC) is rare and unreported. We report a case of EPN in a non-diabetic HCC patient, in which the clinical features of decompensated chronic liver disease masked the more sinister urological emergency to a certain degree. A computed tomography scan of the abdomen revealed a mixture of gas and fluid in the left retrorenal space, in keeping with left EPN. Nevertheless, the course of clinical deterioration was rather rapid that any surgical intervention was not feasible.

Anatomical variations of the celiac trunk are numerous. Variants of the hepatic artery are important to consider especially in the bilio-pancreatic procedures. Unusual arterial anatomy increases the risk of postoperative specific complications. We report a case of a rare anatomic variant of the celiac trunk that gives rise to a left and right hepatic artery separately. It is an exceptional variant found in a patient with a pancreatic head tumor and candidate for duodenopancreatectomy. Key words: celiac trunk, right hepatic artery, anatomic variant.

Splenic artery pseudoaneurysm is a rarely described condition and it is even more rarely considered as a complication of peptic disease. The most common etiologies are pancreatitis, both chronic and acute, and trauma. Diagnosis can be challenging, including ultrasonography, computed tomography scan and angiography, the latter being useful also for therapy (embolization). Given her history of bulimia nervosa and the stress related to hospitalization, our patient was particularly predisposed to peptic ulcer.
