Endometrial response of beef heifers on day 7 following insemination to supraphysiological concentrations of progesterone associated with superovulation

Ovarian stimulation is a routine procedure in assisted reproduction to stimulate the growth of multiple follicles in naturally single-ovulating species including cattle and humans. The aim of this study was to analyze the changes induced in the endometrial transcriptome associated with superovulation in cattle and place these observations in the context of our previous data on changes in the endometrial transcriptome associated with elevated progesterone (P4) concentrations within the physiological range and those changes induced in the embryo due to superovulation. Mean serum P4 concentrations were significantly higher from day 4 to day 7 in superovulated compared with unstimulated control heifers ( P < 0.05). Between-group analysis revealed a clear separation in the overall transcriptional profile of endometria from unstimulated control heifers ( n = 5) compared with superovulated heifers ( n = 5). This was reflected in the number of differentially expressed genes (DEGs) identified between the two groups with 795 up- and 440 downregulated in superovulated endometria. Ten times more genes were altered by superovulation ( n = 1,234) compared with the number altered due to elevated P4 within physiological ranges by insertion of a P4-releasing intravaginal device ( n = 124) with only 22 DEGs common to both models of P4 manipulation. Fewer genes were affected by superovulation in the embryo compared with the endometrium, (443 vs. 1,234 DEGs, respectively), and the manner in which genes were altered was different with 64.5% of genes up- and 35.5% of genes downregulated in the endometrium, compared with the 98.9% of DEGs upregulated in the embryo. In conclusion, superovulation induces significant changes in the transcriptome of the endometrium which are distinct from those in the embryo.

Ovulatory response to the initial gonadotrophin-releasing hormone (GnRH) of the CO-Synch protocol is affected by circulating progesterone (P4) and follicle size. In addition, heifers that ovulate to the initial GnRH treatment have greater fertility after AI. Thus, this study determined the effect of (1) presynchronization (Presynch) before a 6-day CO-Synch protocol and (2) circulating [RCE1] (P4) on ovulatory response, oestrus expression, and pregnancies per AI (P/AI) in beef heifers. Yearling beef heifers (n=233) at three locations were randomly assigned in a 2×2 factorial design to the following treatments: (1) Presynch+6-day CO-Synch with a new P4 device; (2) Presynch+6-day CO-Synch with a once-used P4 device; (3) 6-day CO-Synch with a new P4 device; and (4) 6-day CO-Synch with a once-used P4 device. Presynch consisted of insertion of a new P4 intravaginal device (1.38g of P4) on Day −17 and removal of P4 device on Day −11 concurrently with 500µg of cloprostenol sodium (PGF). On Day −9, all heifers received either a new (New) or once-used (Used) CIDR and 100µg of gonadorelin acetate (GnRH). Six days later (Day −3), CIDRs were removed, 1000µg of PGF was administered and an oestrous detection patch applied (Estrotect). At 72h after CIDR removal, AI was performed concurrently with administration of 100µg of GnRH. Pregnancy was determined by transrectal ultrasonography 31 days after AI. A subset of heifers (n=155) was examined on Day −9 and Day −3 by ultrasonography to determine ovulation to Day −9 GnRH. Data were analysed using generalized linear mixed models (SAS 9.4; SAS Institute Inc.). Presynch heifers had larger follicle diameter on Day −9 (12.7±0.3 vs. 10.1±0.3 mm; P&lt;0.001), greater ovulatory response to Day −9 GnRH (82.5%; 66/80 vs. 56%; 42/75; P&lt;0.001), greater expression of oestrus (90.6%; 106/117 vs. 78.4%; 91/116; P&lt;0.02), and earlier oestrus (49.8±1 vs. 53.1±1 h; P&lt;0.01) compared with controls. There was a treatment×CIDR interaction on oestrous expression, such that a lesser (P&lt;0.05) percentage of control heifers with new CIDR expressed oestrus compared with all other groups (Table 1). Heifers with a used CIDR during the 6-day CO-Synch tended (P=0.08) to have greater P/AI (52.1%; 61/117) than those with a new CIDR (40.5%; 47/116). In conclusion, presynchronization before initiation of a 6-day CO-Synch increased follicle diameter, enhanced ovulatory response and oestrous expression, but did not affect fertility. The earlier onset of oestrus in presynchronized heifers suggests that the timing of AI may need to be modified. Table 1. Introduction: Medically assisted reproduction in natural cycle has been investigated, especially in women with poor response to conventional ovarian stimulation, with endometrial receptivity improvement, lower cost and possibility of successive cycles. The disadvantages are: lower profitability per treatment cycle and higher cancellation rate. The aim of this study was to determine the rate of clinical pregnancy in infertile women subjected to medically assisted reproduction in natural cycle.Material and Methods: Retrospective study of 149 medically assisted reproduction without ovarian stimulation of 50 infertile women, between January/2011 and October/2014.Results: The mean age of women undergoing medically assisted reproduction in natural cycle was 36.1 years. Approximately half (46.0%) of the cycles were performed in poor responders. On the day of ovulation trigger, the mean diameter of the follicle was 17.5 mm. Twenty-three cycles (15.4%) were canceled prior to ovulation trigger. In 8 cycles (5.3%), ovulation occurred between ovulation trigger and oocyte retrieval. In the majority of cycles (n = 118; 79.2%) oocyte retrieval was executed, a medically assisted reproduction technique was performed in 71 (47.6%), mostly intracytoplasmic injection. The overall fertilization rate was 77.5%. In 40 cycles (26.8%) there was embryo transfer. The implantation rate and the clinical pregnancy rate by embryo transfer was 35.0% and 25.0%, respectively. Most pregnancies occurred in poor responders, according to Bologna criteria.Discussion: Although the pregnancy rate per cycle started was 6.7%, the rate of clinical pregnancy per embryo transfer is quite satisfactory, being a group of women with unfavorable responses in previous treatments. The relatively high rates of cycle cancellation are mitigated by the greater simplicity and lower cost of these cycles.Conclusion: The results obtained in this study demonstrate that Medically Assisted Reproduction in natural cycle may be an alternative treatment for ovarian stimulation in patients with poor prognosis, whose only alternative would be oocyte donation.
