Complicated crown fracture and crown-root fracture with pulp involvement expose dental pulp to the oral environment. The pulp outcome is often unpredictable because the patient and injury which are related to variables can influence the treatment of choice and the prognosis of the case. This report presents the case of a 4-year-old boy with complicated crown fracture with pulp polyp in the primary right maxillary central incisor (51) and crown-root fracture with pulp involvement in the primary left maxillary central incisor (61), which was treated only 3 months after the tooth injuries. The treatment of choice was extraction of tooth (61) due to a periapical lesion with disruption of the dental follicle of the permanent successor and pulpotomy (MTA) of the tooth (51), because the pulp presented signs of vitality. At the follow-up visits, no clinical, symptomalogical, and radiographic changes were observed until the primary tooth’s exfoliation. However, at 3-year follow-up, the permanent successors showed hypocalcification and the position of the permanent right maxillary central incisors (11) was altered. Besides the conservative and adequate delayed treatment, the sequelae on the permanent successors could not be avoided.

This report describes the clinical procedures involved in the treatment of a complicated crown-root fracture in the maxillary left central incisor with a wide open apex of a 10-year-old male patient, due to fall from his own height. Post-trauma treatment comprised cervical pulpotomy and adhesive tooth fragment reattachment. After 1 year, clinical and radiograph examinations showed pulp necrosis and an associated periapical lesion. Endodontic therapy with calcium hydroxide-base intracanal dressing, root canal filling and orthodontic extrusion were performed. Extrusion was completed within approximately 16 weeks and the tooth was restored with a post-core system and a prosthetic crown. After a 3 years of follow-up, there was no evidence of apical periodontitis and the tooth was satisfactory both esthetically and functionally.

ABSTRACT Crown fractures of the anterior teeth are a common form of dental trauma mainly affecting children and adolescents. One of the options for managing crown fractures is the reattachment of the fractured fragment when the tooth fragment is available with minimal or no violation of the biological width. Reattachment of fractured fragments can provide good esthetics, as it maintains the tooth's original anatomic form, color, and surface texture. It also restores function and is a relatively simple procedure. This case report deals with the esthetic management of a crown-root fracture that was successfully treated with endodontic treatment followed by reattachment of fractured fragment with fiber post. ABSTRACT Traumatic injury to oral cavity with accompanying tooth fracture can be a tragic experience for the patient and is a problem that requires experience, judgment and skill of the dentist. The dental health and appearance marred by an unsightly injury must be restored to normal as soon as possible. Though, root fractures comprise 0.5 to 7% of injuries affecting the permanent dentition, improper and delayed treatment can lead to loss of tooth. This report records a clinical case of intraalveolar horizontal mid root fracture in maxillary right central incisor with subluxation of coronal segment, which was managed endodontically using mineral trioxide aggregate (MTA), as an apical barrier at the fracture site. Maxillary right lateral and left central incisors affected by oblique crown-root fracture which were restored by custom-cast post and crown. After one year follow-up, the teeth were asymptomatic and showed signs of healing apical to the MTA barrier.

The complicated crown-root fracture of young permanent teeth is an uncommon traumatic dental injury that is usually treated in a complex way and is demanding not only for the dentist but even for the treated child. In this case report, we present the conservative treatment of a maxillary central incisor in a 10-year-old boy after a traumatic dental injury. Treatment included partial pulpotomy and adhesive fragment reattachment after reflection of the mucoperiosteal flap. The patient was fully asymptomatic at 24-month follow-up, with an aesthetically acceptable outcome. Vital pulp therapy and adhesive fragment reattachment can be a viable treatment option for complicated crown-root fractures, especially when treating immature permanent teeth.
