The gastrointestinal (GI) tract is the predominant site of extra nodal lymphoma involvement. In the United States (US), gastric lymphoma is the most common extra nodal site of lymphoma. Most of these lesions are either extra nodal marginal zone B cell lymphoma of mucosa associated lymphoid tissue (MALT) type or diffuse large B cell lymphoma (DLBCL). We report a case of diffuse large B-Cell Gastric Lymphoma who initially presented with sore throat, dysphagia and hiccups for a few months. Esophagogastroduodenoscopy showed lower esophageal stenosis and a large, infiltrative, ulcerated, circumferential mass at the gastro esophageal junction and cardia. Histopathology showed diffuse large B cell lymphoma. Positron emission tomography scan showed advanced disease with presence of lymph nodes on both sides of the diaphragm. The patient was considered to have Stage IV gastric lymphoma. Subsequently, he was treated with R-CHOP regimen (rituximab, cyclophosphamide, hydroxydaunorubicin, oncovin (vincristine), and prednisone).

Primary bone lymphoma (PBL) is an uncommon clinical entity and a rare non-Hodgkin's lymphoma presentation. PBL accounts for less than 5% of malignant bone tumours. Gastric lymphoma (PGL) is also rare cancer, but it is the most common site for extranodal non-Hodgkin lymphoma. We report a rare case of concurrent occurrence of bone lymphoma and gastric lymphoma. The patient presented with a slow-growing swelling around the knee joint for two years. Positron emission tomography incidentally picked up an FDG avid lesion in the fundus of the stomach. Tissue specimens of both the lesions were obtained and conformed to have diffuse large B cell lymphoma on histology and immunochemistry. He is on follow up with the oncology department and being treated with an R-CHOP regimen (rituximab, cyclophosphamide, hydroxydaunorubicin, oncovin (vincristine), and prednisone.

Diffuse large B-cell lymphoma (DLBCL) is the most common form of Non-Hodgkin Lymphoma (NHL) in adults. Affecting nearly 7 out of every 100,000 people in the United States annually, this hematogenous neoplasm is known for its aggressiveness and rapid development. Being the most common NHL, it has been divided into several subgroups based on pathogenesis and treatment methods. In particular, subtypes such as germinal center, activated B-cell-like, and primary mediastinal diffuse large B-cell lymphomas  have been divided by their uniqueness of pathology at the cellular level. Knowing the numerous cytokines, inflammatory markers, and other microcellular processes that these lymphomas disrupt can help target an effective therapeutic at the disease.

After the introduction of highly active antiretroviral therapy (HAART), there has been a decrease in the incidence of lymphoma among the HIV-infected population and also significantly improved survival rates. We describe a remarkable case of an HIV-infected patient with advanced stage IV diffuse large B-cell lymphoma (DLBCL), completely regressed with the use of HAART alone. He remained disease-free for 6 years and he achieved cure without chemotherapy. Although several cases of low-grade lymphoma with complete regression were reported, we could not find any case of stage IV high-grade malignant lymphoma with HAART alone in complete remission for over 5 years from our review of the literature. This unique case shows the importance of HAART in improving survival and achieving cure in HIV–high-grade malignant lymphoma.
