Abstract Background: Oral fluid (OF) is an exciting alternative matrix for monitoring drugs of abuse in workplace, clinical toxicology, criminal justice, and driving under the influence of drugs (DUID) programs. During the last 5 years, scientific and technological advances in OF collection, point-of-collection testing devices, and screening and confirmation methods were achieved. Guidelines were proposed for workplace OF testing by the Substance Abuse and Mental Health Services Administration, DUID testing by the European Union’s Driving under the Influence of Drugs, Alcohol and Medicines (DRUID) program, and standardization of DUID research. Although OF testing is now commonplace in many monitoring programs, the greatest current limitation is the scarcity of controlled drug administration studies available to guide interpretation. Content: This review outlines OF testing advantages and limitations, and the progress in OF that has occurred during the last 5 years in collection, screening, confirmation, and interpretation of cannabinoids, opioids, amphetamines, cocaine, and benzodiazepines. We examine controlled drug administration studies, immunoassay and chromatographic methods, collection devices, point-of-collection testing device performance, and recent applications of OF testing. Summary: Substance Abuse and Mental Health Services Administration approval of OF testing was delayed because questions about drug OF disposition were not yet resolved, and collection device performance and testing assays required improvement. Here, we document the many advances achieved in the use of OF. Additional research is needed to identify new biomarkers, determine drug detection windows, characterize OF adulteration techniques, and evaluate analyte stability. Nevertheless, there is no doubt that OF offers multiple advantages as an alternative matrix for drug monitoring and has an important role in DUID, treatment, workplace, and criminal justice programs.

Abstract: Aims: To compare county-level differences in the number of buprenorphine prescribers listed in the publicly available Substance Abuse and Mental Health Services Administration (SAMHSA) Buprenorphine Practitioner Locator and in the Drug Enforcement Administration’s (DEA) Controlled Substance Act (CSA) database and to determine if disparities in access exist in poorer areas with more non-white residents. Design: Cross-sectional study Setting: TexasMeasurements: County-level counts of buprenorphine prescribers were calculated from both the publicly available SAMHSA buprenorphine practitioner locator list and the DEA CSA database. These were then used to estimate the number of providers per 100,000 residents in each county. Regional variation in access to buprenorphine was compared descriptively across the state using poverty data from the US Census and county-level demography from the Texas Demographic Center. Results: This study found 68.8% more X-waivered providers on the DEA CSA database (n=2,622) with at least one provider reported in 125 of 144 counties in the state (49.2%) compared to the SAMHSA Buprenorphine Practitioner Locator (n=1,553) with at least one provider reported in 103 counties (40.5%). This difference was magnified in Texas Public Health Region 11. This is the poorest region of the state (23.7% of residents below the federal poverty line) and contains the most non-white residents 87.4% vs 54.9% (SD: 13.6%). This region had the lowest number of buprenorphine prescribers with 3.8 providers per 100,000 on the DEA CSA database and 2.5 providers per 100,000 in the SAMHSA Buprenorphine Practitioner Locator.Conclusions: The lack of a complete, public registry of buprenorphine prescribers makes it difficult for patients to identify a convenient buprenorphine prescriber and for referring physicians to help their patients access care. This may be especially true in poorer and more diverse areas with fewer buprenorphine prescribers.

OBJETIVO: Divulgar os dados de um estudo transversal randomizado, realizado em 2001, pelo Centro Brasileiro de Informações sobre Drogas Psicotrópicas. MÉTODOS: A população pesquisada neste levantamento incluiu indivíduos com 12-65 anos de idade, residentes nos 107 maiores municípios do Brasil (com mais de 200 mil habitantes), o que representou 27,7% da população brasileira na época, estimada em 169.799.170 habitantes. Foram realizadas no total 8.589 entrevistas. Utilizou-se o questionário Substance Abuse and Mental Health Services Administration, que foi traduzido e adaptado para o uso no Brasil. RESULTADOS: Do total, 41,1% dos entrevistados disseram já ter utilizado produtos derivados de tabaco alguma vez na vida. A prevalência de uso diário de tabaco foi de 17,4% da amostra (20,3% entre os homens e 14,8% entre as mulheres). Observou-se que 9% da população (10,1% entre os homens e 7,9% entre as mulheres) são dependentes da nicotina, segundo os critérios do National Household Surveys on Drug Abuse. CONCLUSÕES: A prevalência do uso diário de tabaco, nos maiores municípios brasileiros, é significativamente menor na presente década do que a prevalência nacional ao final do século passado.

AbstractThe Substance Abuse and Mental Health Services Administration (SAMHSA) in the Department of Health and Human Services offers extensive disaster behavior health resources to assist disaster survivors in preparing for, responding to, and recovering from natural and manmade disasters. One of SAMHSA’s most innovative resources is the SAMHSA Behavioral Health Disaster Response App (SAMHSA Disaster App). The SAMHSA Disaster App prepares behavioral health responders for any type of traumatic event by allowing them to access disaster-related materials and other key resources right on their phone, at the touch of a button. OBJETIVO: Analisar as situações de violência domiciliar ocorridas com o agressor sob efeito do álcool. MÉTODOS: Foi realizado um levantamento domiciliar que incluiu as 108 cidades brasileiras com mais de 200 mil habitantes em 2005. A amostragem foi por conglomerados, estratificada, probabilística e autoponderada, obtida em três estágios de seleção: setores censitários, domicílios e respondentes (população entre 12-65 anos de idade). O instrumento utilizado para obtenção dos dados foi o Substance Abuse and Mental Health Services Administration, com perguntas sobre dados sociodemográficos e uso de drogas psicotrópicas. RESULTADOS: Foram pesquisados 7.939 domicílios. Em 33,5% foi relatado histórico de violência domiciliar, sendo 17,1% com agressores alcoolizados. Os tipos de violência em associação com uso de álcool mais freqüentes foram: discussões direcionadas a pessoas do domicílio (81,8%), escândalos não direcionados a alguém específico (70,9%), ameaça de agressão física (39,5%) e de quebra de objetos (38,7%), agressões físicas (27,8%), com armas (5,5%) e abuso sexual (3,2%). Mais da metade dos agressores era morador do domicílio, 88,8% deles do sexo masculino. A maioria das vítimas era do sexo feminino (63,9%); 33,9% eram esposas e 18,2% filhos. Quanto às reincidências, 14,1% dos casos perduraram por período entre um a cinco anos e em 14,3% ultrapassaram uma década. A maior parte das vítimas (86%) e dos agressores (77,9%) não procurou por ajuda em serviço de saúde e/ou delegacia. CONCLUSÕES: Além da alta proporção de domicílios brasileiros com histórico de violência com agressores alcoolizados, as agressões apresentaram várias especificidades. A baixa procura por ajuda em serviços de saúde/segurança indica a importância da detecção ativa de casos de violência domiciliar.
