Objective: To investigate construct validity and test–retest reliability of the parent-rated Hand-Use-at-Home questionnaire (HUH) in children with neonatal brachial plexus palsy or unilateral cerebral palsy. Design and subjects: For this cross-sectional study, children with neonatal brachial plexus palsy or unilateral cerebral palsy, aged 3–10 years, were eligible. Main measures: The HUH, Pediatric Outcome Data Collection Instrument Upper Extremity Scale (neonatal brachial plexus palsy only), and Children’s Hand-Use Experience Questionnaire (unilateral cerebral palsy only) were completed. The HUH was completed twice in subgroups of both diagnoses. Lesion-extent (indication of involved nerve rootlets in neonatal brachial plexus palsy as confirmed during clinical observation and/or nerve surgery) and Manual Ability Classification System levels (unilateral cerebral palsy) were obtained from the medical records. Spearman correlation coefficients between the HUH and all clinical variables, agreement, standard error of measurement, smallest detectable change and intra-class correlation were calculated. Results: A total of 260 patients participated (neonatal brachial plexus palsy: 181), of which 56 completed the second HUH (neonatal brachial plexus palsy: 16). Median age was 6.9 years for children with neonatal brachial plexus palsy, 116 had C5-C6 lesions. Median age for children with unilateral cerebral palsy was 6.4 years, 33 had Manual Ability Classification System Level II. The HUH correlated moderately with lesion-extent ( rs =−0.5), Pediatric Outcome Data Collection Instrument Upper Extremity Scale ( rs = 0.6) and Children’s Hand-Use Experience Questionnaire ( rs = 0.5) but weakly with Manual Ability Classification System levels ( rs = −0.4). Test–retest reliability was excellent (intra-class correlation2,1 = 0.89, standard error of measurement = 0.599 and smallest detectable change = 1.66 logits) and agreement was good (mean difference HUH1 − HUH2 = 0.06 logits). Conclusion: The HUH showed good construct validity and test–retest reliability in children with neonatal brachial plexus palsy or unilateral cerebral palsy.

Background/Aims The ability to eat and drink is the most important factor in health, survival and longevity. Children with cerebral palsy can suffer from severe dysphagia, which can lead to a high risk of aspiration and choking. As classifying eating and drinking ability in children with cerebral palsy is important in research and treatment, the aim of this study was to determine the cross-cultural validation and reliability of the Persian version of the Eating and Drinking Ability Classification System. Methods After translation procedures, the face validity, content validity, test–retest reliability and interrater reliability of the Eating and Drinking Ability Classification System were evaluated. In total, 130 parents of children with cerebral palsy and 34 therapists participated in the evaluation of the scale's reliability. The 73 boys and 57 girls with cerebral palsy were aged 3–20 years (mean age 4.4 years) and had various types of cerebral palsy. They were classified according to the Eating and Drinking Ability Classification System by both their parents and the therapists. Results The overall results indicated that the words and sentences used were simple, clear, understandable, relevant and necessary. The intraclass correlation coefficients for test–retest reliability for parents, occupational therapists and speech therapists were 0.98, 0.98 and 0.995, respectively. The interrater reliability between parents and occupational therapists was 0.96, between parents and speech therapists was 0.95, and between occupational therapists and speech therapists was 0.985. Conclusions The Persian version of the Eating and Drinking Ability Classification System seems to be valid and reliable. This system may be used to evaluate children with cerebral palsy.

OBJECTIVE Neonatal brachial plexus palsy (NBPP) occurs due to the stretching of the nerves of the brachial plexus before, during, or after delivery. NBPP can resolve spontaneously or become persistent. To determine if nerve surgery is indicated, predicting recovery is necessary but difficult. Historical attempts explored the association of recovery with only clinical and electrodiagnostic examinations. However, no data exist regarding the neonatal and peripartum factors associated with NBPP persistence. METHODS This retrospective cohort study involved all NBPP patients at the University of Michigan between 2005 and 2015. Peripartum and neonatal factors were assessed for their association with persistent NBPP at 1 year, as defined as the presence of musculoskeletal contractures or an active range of motion that deviated from normal by > 10° (shoulder, elbow, hand, and finger ranges of motion were recorded). Standard statistical methods were used. RESULTS Of 382 children with NBPP, 85% had persistent NBPP at 1 year. A wide range of neonatal and peripartum factors was explored. We found that cephalic presentation, induction or augmentation of labor, birth weight > 9 lbs, and the presence of Horner syndrome all significantly increased the odds of persistence at 1 year, while cesarean delivery and Narakas Grade I to II injury significantly reduced the odds of persistence. CONCLUSIONS Peripartum/neonatal factors were identified that significantly altered the odds of having persistent NBPP at 1 year. Combining these peripartum/neonatal factors with previously published clinical examination findings associated with persistence should allow the development of a prediction algorithm. The implementation of this algorithm may allow the earlier recognition of those cases likely to persist and thus enable earlier intervention, which may improve surgical outcomes.
