Regulation of Mandibular Postures: Mechanisms and Clinical Implications

This review argues that (1) the habitual mandibular position is constantly variable and so cannot be considered as a craniomandibular reference point, (2) there is no unique centric relation, (3) mandibular posture greatly depends on head posture, (4) clinical evaluation of the occlusal vertical dimension is mostly empirical, and (5) neither the vertical dimension at rest nor the centric relation can be determined by means of existing instrument-based clinical methods. However, some physiological conditions exist that facilitate the recording of craniomandibular position.

ABSTRACT Aim This clinical report describes a time-saving recording procedure for implant prosthesis. Background In implant prosthodontics, a few procedures have been suggested to record jaw relationships. Record bases with occlusion rims supported by healing abutments, or screw retained bases are the conventional methods used for mounting definitive implant casts in the articulator. Technique This article describes a simple method that enables the clinician to transfer the existing occlusal vertical dimension (OVD) and centric relation (CR) of acceptable complete denture to final fixed prostheses. Advantages and disadvantages of this procedure are also discussed. Conclusion The method described in this article reduces the chair time and is comfortable for both the patient and clinician. Clinical significance In this article, a chair side technique is presented for articulating the definitive implant casts that maintains the established OVD and CR of the patient's existing complete dentures. The occlusal vertical dimension (OVD) refers to the length of the face as determined by the amount of separation of the jaws. Its determination is important for the manufacture of restorations. However, defining the correct occlusal vertical dimension for edentulous patients is one of the most important steps for function and esthetics rehabilitation. Cephalometry is a standardized method of assessing dental and facial proportions and their interrelation. Additionally, cephalometric analysis of the facial vertical dimension can establish an individual pattern for each patient. This analysis should become a permanent part of each patient’s record. Hence, this study presented a case report with the use of cephalometry as an auxiliary tool in the rehabilitation of OVD. Clinical relevance showed that cephalometric analysis can be an accurate and convenient instrument to treatment planning and prognostic of oral rehabilitation. The reader should understand the clinical implications of using cephalometry as a tool in the rehabilitation of OVD.
