We report a case of sacral fracture complicated by an intrasacral meningocele in a 37-year-old male. The patient had a left sacral fracture with bilateral fractures of the superior and inferior pubic rami. The sacrum was fixed with a posterior plate and both superior pubic rami were fixed with an intramedullary screw. However, computed tomography myelogram and magnetic resonance imaging showed the intrasacral meningocele with the leakage of the cerebrospinal fluid into the buttocks and developed delayed union. Reoperation utilizing a strong anterior and posterior internal fixation combined with posterior bone grafting was undertaken and bone union was obtained.

We present a case of solitary vascular tumor on posterior cranial fossa in a 15-year-old female who had a complaint of headache on/off for six months with associated dizziness, nausea, vomiting and vertigo. Well circumscribed solid vascular mass was localized on posterior fossa by computed tomography along with magnetic resonance imaging scan. Abstract Arachnoid cysts of the spinal canal are relatively common lesions that may be either intra- or extradural. These cysts are usually asymptomatic but may produce symptoms by compressing the spinal cord or nerve roots. We report a case in which an intradural thoracic arachnoid cyst became symptomatic after a routine decompressive lumbar laminectomy for spinal stenosis. Myelography revealed no abnormality, although magnetic resonance imaging and computed tomography after myelography demonstrated a mass within the posterior aspect of the thoracic spinal canal associated with anterior displacement and compression of the spinal cord. A change in the flow dynamics of the cerebrospinal fluid probably allowed the development of spinal cord compression due to one of the following: expansion of the cyst, decreased cerebrospinal fluid buffer between the cord and the cyst, or epidural venous engorgement. A concomitant and more cephalad lesion such as an arachnoid cyst should be considered when myelopathic complications arise after lumbar surgery. Magnetic resonance imaging and computed tomography after myelography are useful to demonstrate the additional pathological processes.

Ossification of the ligamentum flavum (OLF) of thoracic spine occurs along with a progressive spine lesion, and is a rare myelopathy in non-Oriental patients. Computed Tomography or Magnetic Resonance imaging of spine are used for diagnosis, and early decompressive laminectomy is the treatment of choice. We report a case of female patient, 73-year-old, who presented progressive and symmetrical paraparesis, and painful tactile hypoesthesia at T4 level with two months of evolution of the case. After OLF diagnosis, with myelopathy on T1-T2 segment, we performed spine decompression with significant functional better results. According to our research and knowledge, this is the third case to be reported on Brazilian medical literature of a patient with non-Oriental descent.
