Dysplasia epiphysealis hemimelica (Trevor's disease) is a rare developmental disorder of epiphyseal growth. A review of the literature reveals that metatarsal involvement has been described in only two cases and in these cases two metatarsals were reported as being only slightly enlarged. This article reports a case of dysplasia epiphysealis hemimelica involving the medial half of the epiphysis of the left fifth metatarsal.

Introduction. Dysplasia epiphysealis hemimelica, also known as Trevor?s disease, is an extremely rare skeletal developmental disorder of unknown etiology, characterized by an osteocartilaginous outgrowth of one or more epiphyses or of a tarsal bone during childhood. Case report. We presented a sporadic case of dysplasia epiphysealis hemimelica developed in the talus. A 6- year-old boy complained of swelling of his right ankle joint, with painful and reduced movements. Radiographies suggested excessive overgrowth of the dorsomedial aspect of the talus. The tumor was surgically excised and the gross and histological findings were consistent with those of osteochondroma. Conclusion. Dysplasia epiphysealis hemimelica is diagnosed by clinical, radiographic and histopathological examination. Early diagnosis is important for the condition to be treated before the deformity becomes disabling.

Trevor’s disease, also known as dysplasia epiphysealis hemimelica, is a rare developmental bone dysplasia, characterized by benign osteocartilaginous overgrowth pathologically indistinguishable from osteochondroma. The lesion arises from epiphysis more so on the medial aspect of long bones resulting in painless or painful deformities and limb length discrepancies. Imaging plays an important role in diagnosis as the features are characteristic in an appropriate demographic background. We report a case of Trevor’s disease in a 2-year male with multifocal involvement and characteristic imaging findings.
