Purpose The purpose of this investigation was to systematically examine outcomes associated with Semantic feature analysis, which is an established treatment for word-retrieval deficits in aphasia. Attributes of the experimental design and stimuli were manipulated to evaluate generalized naming of semantically related and unrelated items. In addition, the study was designed to examine changes in production of semantic information. Method Semantic feature analysis was applied in the context of multiple-baseline designs with 5 persons with chronic aphasia. Experimental items were controlled for semantic category membership, number of naming attempts, and provision of item names. Acquisition, generalization, and maintenance effects were measured in probes of naming performance. Production of semantic information was also measured in response to experimental items and in discourse tasks. Results Treatment was associated with systematic increases in naming of trained items for 4 of the 5 participants. Positive generalization to untrained exemplars of trained categories was found for repeatedly exposed items but not for limited-exposure items. Slight increases in production of semantic content were observed. Conclusion Repeated attempts to name untreated items appeared to play a role in generalization. Provision of the names of untrained items may have enhanced generalized responding for 2 participants.

Word retrieval deficit is found to be one of the most persistent symptoms reported among the constellation of symptoms exhibited by persons with aphasia (PWAs). This deficit restraints the persons with aphasia to perform with ease across day-to-day conversations. As a consequence, PWAs fail to communicate their desired ideas or thoughts. Word retrieval is an intricate process as it entails various levels of processing. In addition, word retrieval breakdown can occur at multiple levels (semantic level or lexical-semantic level, or phonological level). Thus, there is a need for speech-language pathologists (SLPs) to treat this deficit through effective treatment approaches. In recent decades, semantic feature analysis, verb network strengthening treatment, and phonological component analysis have received greater focus and importance in treating word retrieval deficits. Many studies confirmed that the use of these treatment approaches on PWAs possesses a pivotal role in remediating word retrieval deficits.

Primary progressive aphasia (PPA) is a neurodegenerative condition characterized by language and cognitive decline. Word-retrieval deficits are the most common PPA symptom and contribute to impaired spoken expression. Intense semantic interventions show promise for improving word retrieval in people with PPA. In addition, people with PPA may learn to use alternative communication modalities when they are unable to retrieve a word. However, executive function impairments can cause people to struggle to switch among modalities to repair communication breakdowns.This study examined the effects of a combined semantic feature analysis and multimodal communication program (SFA+MCP) on word-retrieval accuracy, switching among modalities, and overall communicative effectiveness in a person with PPA. An adult female with PPA completed SFA+MCP. Baseline, probe, intervention, and postintervention sessions were completed to measure word-retrieval accuracy and switching between communication modalities. A postintervention listener task was completed to measure communicative effectiveness. Changes in word-retrieval accuracy and switching were minimal. However, the listeners’ identification of the participant’s communication attempts was more accurate following treatment, suggesting increased overall communicative effectiveness. Further investigations of SFA+MCP, specifically relative to timing, intensity, and appropriate modifications for people with cognitive impairments associated with PPA are warranted.

Purpose This investigation was designed to examine the generalization effects of semantic treatment for word retrieval deficits in people with aphasia. Semantic feature analysis (SFA; Boyle & Coelho, 1995), typicality treatment (Kiran & Thompson, 2003), and mediating strategy training were combined to maximize potential generalization effects. Method Treatment, which included SFA and a semantic feature judgment task, was conducted with 9 participants with chronic aphasia in the context of multiple baseline designs across behaviors. Typical and atypical exemplars were trained across animate and inanimate categories. Treatment was sequentially modified to overtly train the use of SFA as a mediating strategy. Results Eight of the 9 participants demonstrated improvements in naming of trained stimuli. Positive generalization effects were limited overall; possible response generalization was evident for 5 participants. Instruction in the use of a mediating strategy resulted in improved naming of treated words for all participants; however, generalization to untreated words did not occur. Conclusion Treatment using SFA resulted in improved naming of treated typical and atypical exemplars in both animate and inanimate categories for 8 of 9 participants. Training in a mediating strategy also resulted in improved retrieval of experimental words. Regardless of intervention approach, generalization to untreated items was limited.

Purpose The ultimate goal of anomia treatment should be to achieve gains in exemplars trained in the therapy session, as well as generalization to untrained exemplars and contexts. The purpose of this study was to test the efficacy of phonomotor treatment, a treatment focusing on enhancement of phonological sequence knowledge, against semantic feature analysis (SFA), a lexical-semantic therapy that focuses on enhancement of semantic knowledge and is well known and commonly used to treat anomia in aphasia. Method In a between-groups randomized controlled trial, 58 persons with aphasia characterized by anomia and phonological dysfunction were randomized to receive 56–60 hr of intensively delivered treatment over 6 weeks with testing pretreatment, posttreatment, and 3 months posttreatment termination. Results There was no significant between-groups difference on the primary outcome measure (untrained nouns phonologically and semantically unrelated to each treatment) at 3 months posttreatment. Significant within-group immediately posttreatment acquisition effects for confrontation naming and response latency were observed for both groups. Treatment-specific generalization effects for confrontation naming were observed for both groups immediately and 3 months posttreatment; a significant decrease in response latency was observed at both time points for the SFA group only. Finally, significant within-group differences on the Comprehensive Aphasia Test–Disability Questionnaire ( Swinburn, Porter, & Howard, 2004 ) were observed both immediately and 3 months posttreatment for the SFA group, and significant within-group differences on the Functional Outcome Questionnaire ( Glueckauf et al., 2003 ) were found for both treatment groups 3 months posttreatment. Discussion Our results are consistent with those of prior studies that have shown that SFA treatment and phonomotor treatment generalize to untrained words that share features (semantic or phonological sequence, respectively) with the training set. However, they show that there is no significant generalization to untrained words that do not share semantic features or phonological sequence features.
