The discontinuation of life sustaining medical treatment (LSMT) in severely and permanently impaired neonates, especially artificial nutrition and hydration (ANH) is subject to uncertainty and controversy. Definitive clinical guidelines are lacking, clinical research is limited, ethical disagreement is commonplace, and while case and statutory law provide legal underpinning for the practice in defined circumstances, uncertainty in this realm likely influences clinical practice. We use the case of a neurologically devastated neonate to highlight and review these arenas, and show how, using available legal, ethical, and clinical standards and practice, the case of Baby O was resolved, and to underline the need for further research in neonatal palliative care.

The ongoing coronavirus disease 2019 (COVID-19) pandemic forced a change in the way we provide medical treatment. Endocrinology in the era of COVID-19 had to transform and reduce its vast potential to the absolute necessities. Medical professionals needed to update their clinical practice to provide their patients as much support and as little harm as possible in these increasingly difficult times. International expert statements were published to offer guidance regarding proper care. It was suggested to simplify the diagnostic scheme of hypercortisolemia and to modify the approach to treatment. Hypercortisolemic patients with COVID-19 and iatrogenic hypercortisolemia due to glucocorticoid use are important clinical scenarios – we aimed to provide a cohesive summary of issues to consider.

Healing people and medical care are together highly organized technological system with significant expert, ethical and legal regulative. Taking medical care is very sensitive area and it interfears deep into one?s integrity, so the law is necessary in this area as a regulator. The aim of work is to show medical errors from legal aspects and clinical practice. Errors, negligent conduct during the medical treatment and bad results of medical treatment are categories that can easily be switched or can sublime themselves into the same thing. That is why correct differentiation of medical errors and viewing every way of medical errors appearance is necessary.

Forgoing life sustaining medical treatment takes on additional complexities depending on the mode of therapy forgone, which often varies in clinical practice. Compassionate extubation of the paralyzed or alert patient requires more in-depth ethical analysis. Some cardiologists are reluctant to deactivate pacemakers, requiring thoughtful comparison of these devices with implantable defibrillators, which do not prompt such debate. Dialysis is a burdensome intervention that may not prolong life and when instituted should be considered a time-limited trial. Enteral and parenteral nutrition create distinct issues in terms of emotional connotation, burden, and benefit.
