Summary Aesthetic outcome of implant therapy involves the appropriate architecture of peri-implant soft tissue and interdental papilla. The dynamic compression technique of soft tissue is mentioned in contemporary literature as one of the methods for achieving optimal emergence profile. The aim of this case report was to present soft tissue prosthetic modeling with temporary crowns on implants for obtaining an emergence profile of final restoration. A 25-year-old female patient with missing left maxillary central incisor was referred to the Department of Oral Surgery School of Dental Medicine University in Belgrade for dental implant placement. After detailed planning, implant placement of Straumann Bone level implant NC Ø 3, 3×12 mm (Straumann® Bone Level NC), in the position of 21 was performed. Upon completion of osseointegration period, screw retained laboratory temporary crown was fabricated. During the next 3 months emergence profile was scalloped by creating additional pressure on the soft tissue with periodic adding the composite resin material to a temporary crown. The created emergence profile was transferred to the master cast by using customized impression coping, making possible fabrication of the final implant restoration according to the design made with provisional restoration. Soft tissue conditioning using temporary dental restorations on implants presents a non-invasive method with predictable aesthetic result.

The nature of immediate implant placement followed by an immediate restoration protocol makes it particularly suited to the anterior maxilla. In addition to saving treatment time and avoiding additional surgical procedures, this protocol has been reported to improve aesthetic outcomes by supporting the peri-implant tissues during the implant healing phase through the use of a provisional restoration. This case report documents the use of this protocol in a patient with a failing maxillary anterior tooth and reports on the soft and hard tissue changes over an observation period of 10 years. An implant was immediately placed after removal of a failing maxillary central incisor followed by the provision of a screw retained provisional crown on the same day. A definitive restoration was placed after a 3-month healing period. Not only did this protocol manage to maintain peri-implant bone levels over the 10-year follow-up period, excellent aesthetic outcomes and very limited soft tissue recession were observed with the use of this technique.

Abstract This article describes a newly designed surgical template that was used to facilitate dental implant placement. The implants were planned to function by loading them immediately. A case report describing the device and the benefits of its use for a patient with an edentulous mandible is presented. Four implants were placed in the anterior region of the mandible to support an immediately fixed prosthesis. Clinical and radiographic analyses were conducted postoperatively to evaluate bone loss and peri-implant soft-tissue healing. The salutary results demonstrated the positive value of this therapeutic approach and presented the advantages of shorter treatment times, fewer patient visits, lower costs, and elimination of secondary surgical procedures.

Aim The aim of this prospective study was to determine the influence of vertical soft tissue thickness on bone level changes in platform-switched implants placed eqicrestally or subcrestally and restored with screw-retained or cement-retained restorations. Methods Platform-switched bone-level implants were placed in a single stage manner in the posterior mandibular region. Implant sites were divided into thick (control) and thin (test) vertical soft tissue groups. The implants in the control group were placed equicrestally. The implant sites from the control group were randomly allocated to receive equicrestally or subcrestally placed implants. Bone remodeling/loss was radiographically measured at baseline, three months postoperatively and six months after delivery of final prosthetic restoration. Results The mean crestal bone loss values three months postoperatively and six months post prosthetic restoration were higher in sites with thin versus sites with thick gingiva. In implant sites with thin gingiva, subcrestally placed implants presented less bone loss than eqicrestally placed implants. Conclusion Platform switched implants are prone to more bone loss when they are placed in sites with thin soft tissue, regardless of the type of final restoration (screw-retained or cement-retained). Subcrestal placement of platform-switched implants can prevent crestal bone loss in sites with vertical soft tissue thickness < 3 mm.

Objective. This case report describes soft-tissue management after a failed GBR procedure to enhance the soft-tissue quality, quantity, and aesthetic outcomes. Case Presentation. A 38-year-old patient visited the Dental Clinic of the Ospedale Maggiore Policlinico, University of Milan, Milan, Italy, for a chief complaint of unsatisfactory aesthetics of the anterior maxillary area. Dental history disclosed failed preimplant vertical bone augmentation with GBR procedure in the area of the left maxillary central incisor resulting in a severe gingival recession of adjacent teeth and compromised soft-tissue quantity and quality and severe ridge atrophy (class III). Multilayered connective tissue grafting technique, in a two-step mucogingival surgery, was used to cover the gingival recessions, reach even gingival margin, and reconstruct the defect. Frenectomy was done after the second mucogingival surgery to relieve the muscle attachment. A definitive cantilever bridge was placed to restore the left and right maxillary central incisors, and a satisfactory aesthetic outcome was reached. Conclusions. Multilayered connective tissue grafting technique might be successful in correcting soft-tissue quantity and quality in class III ridge defects.
