Uterine torsion is defined as a rotation of uterus more than 45 degrees along its long axis. However, a pathologic rotation of the uterus beyond 45 degrees-torsion of the entire uterus-is rarely seen in obstetrical practice, authors report a case of torsion of the uterus by 90 degrees. The patient, a 30-year-old gravida 3 para 2 at 37 weeks’ gestation with a singleton pregnancy, her prior obstetrical history included two uncomplicated term vaginal deliveries, and the current pregnancy had been uncomplicated until the date of presentation was admitted to the obstetrical unit  with  labour at 37 weeks 5 days ,on obstetric examination the patient was in labour with transversal presentation of the fetus so an emergency caesarean section (CS) was carried out for. At the time of CS, the diagnosis of uterine torsion of 90 degrees was made. After the delivery of the baby, uterus returned to anatomical position and the torsion corrected spontaneously. The patient recovered and was discharged home with her baby on the third postoperative day. Uterine torsion is an infrequently reported and potentially dangerous complication of pregnancy that occurs mainly in the third trimester.

Uterine torsion is considered an obstetrical emergency and should be corrected as soon as it is diagnosed. It occurs frequently in buffaloes as compared to other species of animals, mainly due to their peculiar anatomy and physiology. This case report describes the occurrence of uterine torsion in a pregnant pluriparous buffalo. The buffalo was presented with history of persistent straining for over 6 hours. The cervix was fully dilated and obstetrical examination revealed dystocia due to a left sided uterine torsion. Haematological results revealed a normal PCV but with an accompanying macrocytosis. There was also neutrophilia, eosinophilia as well as lymphopenia as well as hypocalcium and hypomagnesemia. The dystocia was corrected using the Schafer’s technique. This involved the placing of the free end of a wooden plank on the left flank while the other free end, having an assistant standing on it rested on the ground. Rolling the buffalo twice with this arrangement corrected the anomaly.

In uterine torsion, the uterus twists more than 45 degrees around its long axis at the junction between thecervix and the corpus. Probably due to rarity only few cases are reported where detorsion was either notpossible or posterior uterine wall opening, more tactfully incisions inflicted transversely, have been the onlyway out.Reported herewith is a 26 yr, third gravida at 38 weeks pregnancy, with previous caesarean in early labourand underwent cesarean for rupture of the membrane, was realized to have an incision inflicted in theposterior wall of the uterus, 3 cm above the disposition of uterosacral ligament amidst severe torsion of theuterus that was successfully detorted avoiding hysterectomy, in view of the young age.Key words: uterine torsion; term pregnancy complication; uterine rotation.DOI: 10.3126/njog.v4i1.3335Nepal Journal of Obstetrics and Gynaecology June-July 2009; 4(1): 55-57
