Abstract The use of ultraviolet light (UV) from low pressure mercury lamps for destroying iron cyanide in synthetic and actual gold mill effluents was evaluated in this study. For the light intensities used in this study, UV irradiation was not able to efficiently destroy cyanide. However, it converted iron cyanide to a weak acid dissociable form which was destroyed by chlorine. Data from several bench-scale tests and one pilot scale test were used to estimate quantum efficiencies (moles iron cyanide destroyed/einstein). These efficiencies ranged from 0.2% to 1%; approximately 30% to 90% lower than those reported in the literature for potassium ferricyanide. The data collected during the study demonstrated the technical feasibility of using UV in conjunction with chlorination for destroying iron cyanide in gold mill effluents. However, low pressure mercury lamps do not appear to be a practical UV source for this purpose. Irradiation with high intensity lamps may be more practical and is recommended for experimental evaluation.

Abstract Background Although patients who work and have related health issues are usually first seen in primary care, providers in these settings do not routinely ask questions about work. Guidelines to help manage such patients are rarely used in primary care. Electronic health record (EHR) systems with worker health clinical decision support (CDS) tools have potential for assisting these practices. Objective This study aimed to identify the need for, and barriers and facilitators related to, implementation of CDS tools for the clinical management of working patients in a variety of primary care settings. Methods We used a qualitative design that included analysis of interview transcripts and observational field notes from 10 clinics in five organizations. Results We interviewed 83 providers, staff members, managers, informatics and information technology experts, and leaders and spent 35 hours observing. We identified eight themes in four categories related to CDS for worker health (operational issues, usefulness of proposed CDS, effort and time-related issues, and topic-specific issues). These categories were classified as facilitators or barriers to the use of the CDS tools. Facilitators related to operational issues include current technical feasibility and new work patterns associated with the coordinated care model. Facilitators concerning usefulness include users' need for awareness and evidence-based tools, appropriateness of the proposed CDS for their patients, and the benefits of population health data. Barriers that are effort-related include additional time this proposed CDS might take, and other pressing organizational priorities. Barriers that are topic-specific include sensitive issues related to health and work and the complexities of information about work. Conclusion We discovered several themes not previously described that can guide future CDS development: technical feasibility of the proposed CDS within commercial EHRs, the sensitive nature of some CDS content, and the need to assist the entire health care team in managing worker health.

AbstractPhotodynamic diagnostic (PDD) is an optical enhancement option for the endoscope to support the detection of cancer, for example in the bladder. In real application PDD efficiency suffers due to the complex accumulation of the photosensitizing drug inside the tumor and the associated processes of heme syntheses to create the fluorescent components needed. To optimize the diagnostic outcome of PDD it would be helpful to predict the optimal time for diagnosis based on measurable precursors. In a previous cell study, we proposed a new filter fluorometer to image the accumulation of the precursors Coproporphyrin III (CP-III) and Uroporphyrin III (UP-III) that metabolize to Protoporphyrin IX (PP-IX) later. This accumulation process can be used to predict the optimal time slot for diagnostic imaging. Therefore, a new filter system was designed to distinguish between CP-III and PP-IX. In this work we tested this filter system in combination with a standard PDD endoscopic imaging system. Goal of this study was to prove the technical feasibility in a non-patient setup to prepare a later clinical study.
