Abstract The authors report a case of cranial plasmacytoma with multiple myelomas and palsy of the lower cranial nerves. The osteolytic lesion adjacent to the jugular foramen was demonstrated by an angiogram to be exceedingly hypervascular, with arteriovenous shunting resembling that seen in paragangliomas. Forty-five cases of cranial and intracranial plasmacytoma from the literature were reviewed. The findings indicate that a cranial plasmacytoma commonly appears to be a hypervascular tumor, whereas most dural tumors or intraparenchymal tumors have poor vascularity.

Background: Foramen magnum meningiomas are challenging tumors, requiring special considerations because of the vicinity of the medulla oblongata, the lower cranial nerves, and the vertebral artery. After detailing the relevant anatomy of the foramen magnum area, we will explain our classification system based on the compartment of development, the dural insertion, and the relation to the vertebral artery. Method: A 35 years age lady with foramen magnum meningioma was operated at the Department of Neurosurgery, Bangabandhu Sheikh Mujib Medical University 6 months back. A case report was taken before surgery and 6 month follow up was done after operation. Conclusion: As foramen magnum meningioma is very difficult to approach, with the help of microscope with proper training. AbstractSchwannomatosis is a distinct syndrome characterized by multiple peripheral nerve schwannomas that can be sporadic or familial in nature. Cases affecting the lower cranial nerves are infrequent. Here, the authors present a rare case of schwannomatosis affecting the left spinal accessory nerve. Upon genetic screening, an in-frame insertion at codon p.R177 of the Sox 10 gene was observed. There were no identifiable alterations in NF1, NF2, LZTR1, and SMARCB1. This case demonstrates a rare clinical presentation of schwannomatosis in addition to a genetic aberration that has not been previously reported in this disease context.

Introduction: Choroid plexus tumors (CPTs) are rare papillary neoplasms derived from choroid plexus epithelium. They account for only approximately 0.4%-0.6% of all intracranial tumors, but 10%-20% of brain tumors occurring throughout the first year of life. Objective: The present study describes the case of an adult man presenting a cerebellopontine angle choroid plexus papilloma (CPP) microsurgically treated through suboccipital far-lateral approach. Case report: A 67-years-old-man was admitted presenting progressive headaches and left lower limb weakness. Magnetic resonance images showed a large tumor on the left cerebellopontine angle with heterogeneous contrast enhancement. Total surgical resection was achieved through a right suboccipital far-lateral craniotomy on lateral position with neurophysiological monitoring of lower cranial nerves. No alterations on cranial nerves function was observed during tumor resection. The patient presented an uneventful recovery and was discharged home on the fifth post-operative day. On the six months follow-up, he was asymptomatic. Anatomopathological analysis confirmed the diagnosis of CPP, WHO grade I. Conclusion: Cerebellopontine angle’s cpp is an extremely rare neoplasm that challenges a neurosurgeon ability to deal with tumor in close relation to lower cranial nerves. Appropriate neurosurgical route, surgeons experience and adequate anatomical knowledge of important neural and vascular structures are fundamental to safely remove CPP of the posterior fossa. Suboccipital far-lateral craniotomy gives enough view with minimal retraction to manage the tumor.

Abstract Objective Foramen magnum meningiomas present a formidable challenge to resection due to frequent involvement of the lower cranial nerves and vertebrobasilar circulation. The video shows the use of a far lateral craniotomy to resect a foramen magnum meningioma. Design, Setting, and Participant A 49-year-old woman presented with neck pain and was found to have a large foramen magnum meningioma (Fig. 1A, B). Drilling of the posterior occipital condyle was required to gain access to the lateral aspect of the brain stem. The amount of occipital condyle resection varies by patient and pathology. Outcome/Result Maximal total resection of the tumor was achieved (Fig. 1B, C), and the patient was discharged on postoperative day 4 with no neurologic deficits. The technique for tumor microdissection (Fig. 2) is shown in the video. Conclusion Given the close proximity of foramen magnum meningiomas to vital structures at the craniocervical junction, surgical resection with careful microdissection and preservation of the overlying dura to prevent postoperative pseudomeningocele is necessary to successfully manage this pathology in those patients who are surgical candidates.The link to the video can be found at: https://youtu.be/Mds9N1x2zE0.
