The localized form of pigmented villo-nodular synovitis (PVNS) is a rare pathological entity characterized by limited involvement of the synovium. The knee is the most commonly affected joint, and the disorder presents as a nodular, pedunculated lesion. The lesion usually presents as mechanical symptom such as locking of the knee. We report a case of localized PVNS that involved patellar fat pad an extremely rare area of involvement, previously described only in 4 cases, presenting as palpable mass. Complete excision of the lesion was performed through superolateral portal with use of 70-degree arthroscope. 70-degree arthroscope allows us to completely visualize infra-patellar region through superolateral portal and is recommended than 30-degree arthroscope for treatment of infra-patellar lesions. Patient was symptomless at 3 months follow up.

Ganglion cyst of the knee joint can be intra articular or extra articular. Large intra articular ganglionic cystic formations arising from the infrapatellar fat pad are quite uncommon and only few are reported. Treatment option is surgery(open/arthroscopic) depending on the site and size of the cyst. We report a case of giant ganglion cyst arising from the infra patellar Hoffa's fat pad which presents anterior to the patellar tendon through a rent in the medial retinaculum with the posterior wall of the cyst being adherent to the para tenon of the patellar tendon, in a 58 year old male which presents anterior to the patellar tendon through a rent in the patellar retinaculum which was adherent to the para tenon of the patellar tendon and it was managed by open excision of the cyst.

The periodontal treatment of teeth with furcation defect is clinically challenging. In cases of class II furcation defects, the regenerative surgery shows low morbidity and good prognosis when correctly indicated. The aim of the presentstudy is to report a treatment option for class II furcation defect through autogenous bone graft associated with the Bichat’s fat pad. Case report: A 59-year-old female patient was diagnosed with class II furcation defect in the left mandibular first molar. The treatment comprised surgical reconstruction of the defect with a combination of maxillary tuberosity bone graft and Bichat’s fat pad. The clinical and radiographic follow-up of 180 days showed bone formation inthe furcation area and absence of probing depth. Conclusion: An association of autogenous graft form the maxillary tuberosity with a Bichat’s fat pad proved to be a safe, low cost, and effective therapy for the regenerative treatment of class II furcation.
