Calcium pyrophosphate dihydrate crystal deposition disease has various clinical features, and pseudogout is one of the six clinical forms. Chondrocalcinosis is the term used to describe the radiographic appearance of the disease. A review of the literature revealed that the appearance of this type of arthropathy in the foot is infrequent. We offer a review of the disease and report an atypical bilateral case of pseudogout in a patient 56 years of age without a history who presented with symptoms of arthritis localized in the first metatarsophalangeal joint associated with hallux valgus and was treated surgically. Radiographic evaluation of the feet did not reveal signs of chondrocalcinosis. The patient had no metabolic abnormalities, except for high uric acid values. Chemical analysis of the surgical samples demonstrated the presence of calcium pyrophosphate dihydrate crystals, confirming the diagnosis. We believe that arthropathy by deposition of calcium pyrophosphate dihydrate in the foot, although rare, must be considered in the podiatric physician’s differential diagnosis when a patient presents with articular pain in the foot associated or not with deformities. We dissected 20 preserved Caucasian cadaveric upper limbs looking at the relation of the superficial branch of the radial nerve (SBRN) to the brachioradialis tendon. SBRN emerged from deep to superficial position by piercing the brachioradialis tendon near its dorsal border in four limbs. The resulting dorsal tendinous band compressed the nerve and prevented longitudinal gliding movement during ulnar flexion. This is likely to increase the risk of chronic compression neuropathy (Wartenberg's syndrome). In two of these four limbs, there was a communication between the SBRN and lateral cutaneous nerve of the forearm. No such communication was found in the remaining 16 forearms. This communication could contribute to the minimal area of sensory loss observed in Wartenberg's syndrome. We recommend that this anatomical anomaly is looked for and if present dealt with during surgical treatment of Wartenberg's syndrome, as it is likely to predispose to chronic compression neuropathy.

The authors report on 3 rare cases of ruptured lenticulostriate artery (LSA) aneurysms that were heralded by deep cerebral hematomas. The hematomas were unilateral in 2 cases and bilateral in 1; in the bilateral case, only a single LSA aneurysm could be identified on the right side of the brain. Because of their small size (≤ 2 mm), fusiform aspect, and deep location within the brain, all of the aneurysms were treated conservatively. There was no hemorrhage recurrence, and follow-up angiography demonstrated spontaneous thrombosis in 2 of the 3 cases. The clinical course was favorable in 2 of the 3 patients. The course in the patient with the bilateral hematoma was marked by an ischemic event after the initial episode, resulting in an aggravation of deficits. The cause of this second event was uncertain. Because our knowledge about the natural history of LSA aneurysms is incomplete, there is no consensus concerning a therapeutic strategy. The authors' experience in 3 reported cases leads them to think that a conservative approach involving close angiographic monitoring may be proposed as first-line treatment. If the monitored aneurysm then persists or grows in size, its occlusion should be considered. Nonetheless, other studies are needed to further strengthen the legitimacy of this strategy.

Stimuli in different parts of the visual field can be perceived as independent entities and as conjoined wholes. It is of interest to determine whether there are cortical representations of the left and right hemifields which remain as independent entities when both hemifields are stimulated simultaneously and/or whether they interact to form a conjoined representation. We examined whether cortical processing of visual stimuli depends on whether they occur in isolation in one hemifield (unilaterally) or simultaneously in both hemifields (bilaterally). Visual evoked potentials of six normal subjects were recorded from 128 scalp sites. Wedge-shaped chequerboard stimuli, extending 1 – 4 deg eccentricity, were presented to quadrants of the visual field. Stimulus duration was 250 ms; the stimulus onset asynchrony was random, 500 – 750 ms. The evoked potentials revealed multiple peaks of activity with different surface topography. Prominent deflections occurred around 80, 120 – 180, and 230 ms. The response to bilateral stimuli was compared with the sum of the responses to unilateral stimuli. On the basis of the multichannel recordings, nonlinear interactions were characterised as either (a) modulations (same generators, but different amplitude) or (b) interactions originating from different generators. Modulation occurred at 230 ms, the response being suppressed for the bilateral case. At 120 – 180 ms, the field patterns suggested that at least some of the sources of the interaction effect are different from the source of the bilateral response. Underlying generators of the evoked responses and the interaction effects were further explored with the use of an equivalent current dipole model.
