Direct anterior resin composite bridge – a case report

Background: Dental hard tissue loss renders a tooth restorable or unrestorable. The treatment ranges from endodontic treatment to dental implant. Sometimes patients in need to restore their oral function and esthetics cannot afford those treatments due to a limited financial capability and time availability. Purpose: This case report presents a minimal invasive, single appointment, quick, and affordable alternative treatment to restore morphology, function, and esthetics. Case: A 40 years old male patient suffered enamel-dentin-pulp fracture on central incisor due to trauma. Eventhough the fracture involves pulp exposure, it remains vital. The tooth lacks sound tooth structure for ferrule effect. The patient wished to retain the tooth. Therefore, initial treatment plan comprised of reestablishing ferrule effect, root canal treatment, endodontic post, and porcelain crown. Nevertheless, due to financial and time constraint, patient refused the suggested treatment plan; thus, direct resin composite bridge was suggested. Case Management: Isolation is followed by calcium hydroxide capping of the exposed pulp. After application of etch and adhesive bonding to tooth 11, 21, and 22, nanohybrid resin composite was layered to fabricate the direct composite bridge, bonded to 11 and 21, with the following layering sequence: 1) palatal; 2) proximal; 3) body; 4) labial. The procedure took 1 hour to restore tooth morphology, function, and esthetics. Upon five years follow-up, the tooth remains asymptomatic. The direct resin composite bridge has been serving well. Conclusion: Direct resin composite bridges can be an affordable, quick, and minimal invasive treatment modality resulting in satisfactory esthetics, function, and longevity.

No- or minimal-preparation veneers associated with enamel preservation offer predictable results in esthetic dentistry; indirect additive anterior composite restorations represent a quick, minimally invasive, inexpensive, and repairable option for a smile enhancement treatment plan. Current laboratory techniques associated with a strict clinical protocol satisfy patients’ restorative and esthetic needs. The case report presented describes minimal invasive treatment of four upper incisors with laminate nanohybrid resin composite veneers. A step-by-step protocol is proposed for diagnostic evaluation, mock-up fabrication and trial, teeth preparation and impression, and adhesive cementation. The resolution of initial esthetic issues, patient satisfaction, and nice integration of indirect restorations confirmed the success of this anterior dentition rehabilitation.

Clinical Relevance Adhesive restorations allow clinicians the utilization of significantly more conservative tooth preparation, thus preserving and reinforcing sound tooth structure. In selected clinical cases, these advantages can be used and expanded for extensive restorations.

Cosmetic veneer has become a popular option for patients seeking to improve aesthetic in their smile. The procedure can be done through a direct or indirect technique which differs in term of material used and laboratory involvement. Despite producing a better aesthetic result than its direct technique counterpart, high-cost treatment, multiple visit requirement and invasive nature of tooth preparation are proven to be a stumbling block to the patient in some cases to choose indirect technique. Therefore, direct composite veneer technique can be offered due to its low costing and conservative concept which advocates more tooth structure preservation. This case report is about direct composite veneer for anterior teeth rehabilitation and its simple and conservative protocol.

Diagnosis and treatment planning of untreated traumatised anterior teeth specially in children with psychiatric disorders or special health care needs is a challenge to paediatric dentist, that too in non-pharmacological means is a difficult task. Most of the time when these patients report with such complaint, it is already too late. This could be due to unintentional reasons like lack of parent awareness or patient’s response at the time of insult. All these factors bypass the time of conservative and minimal invasive treatment approaches. Ultimately, delayed treatment of trauma to anterior region exposes pulp and infection progress to periradicular region with cystic changes. Here, in this case report, the non-pharmacological management of periradicular cyst enucleation, followed by apicoectomy and Platelet Rich Fibrin (PRF) grafting was done in a 15-year-old female patient with obsessive compulsive disorder i.e., Trichotillomania (TTM) under local anaesthesia. Symptoms and intraoral swelling was resolved postoperatively in follow-up visits.

<p><strong> </strong>This case report illustrates a conservative technique for reintervention on anterior composite restorations in a young patient with composite veneers. The treatment incorporated the use of photos and digital planning to establish a harmonious smile design. In order to conserve the dental tissues, existing resin composites restorations in maxillary anterior teeth were carefully removed to minimize enamel reduction, followed by a study of color and shade of teeth considering factors such as hue, chroma, luminosity, opacity, translucency and light effects. Eight direct veneers were built up with resin composite taking the optical properties of the material into account and their influence on the thickness of the layers that create specific light effects when superimposed on each other. To obtain excellent results with a predictable working protocol, nanohybrid composite systems facilitate good aesthetics, avoiding more invasive treatment in an economic manner.</p><p><strong>Keywords</strong></p><p>Dental aesthetic; Optical proprieties; Resin composite.</p>
