SUMMARY Objective: This in vitro study aimed to evaluate the preservation of enamel after tooth preparation for porcelain laminate veneers (PLVs) at different preparation depths based on a fully digital workflow. Methods and Materials: Sixty extracted human maxillary anterior teeth, including 20 maxillary central incisors (MCIs), 20 maxillary lateral incisors (MLIs), and 20 maxillary canines (MCs) underwent microcomputed tomography (CT) scanning, and were reconstructed as three-dimensional (3D) enamel and dentin models. Subsequently, the three-dimensional (3D) enamel models were imported into Materialise, where each enamel model underwent seven types of virtual preparation for PLVs at preparation depths at 0.1-mm increments from 0.1-0.3-0.5 mm (D1) to 0.7-0.9-1.1 mm (D7). The enamel surface was depicted by merging the virtual preparation and, respective, dentin models. The enamel area and prepared surface were measured to calculate the percentage of enamel (R%). The data were statistically analyzed using one-way analysis of variance (ANOVA) (α=0.05). Results: The group-wise mean (standard deviation) R values for the MCIs were as follows: D1-D3: 100.00 (0) each, and D4-D7: 74.70 (2.45), 51.40 (5.12), 24.40 (3.06), and 0.00 (0), respectively. The group-wise mean R values for the MLIs were 100.00 (0), 73.70 (3.40), 53.50 (3.44), 25.20 (3.79), and 0.90 (0.99) for the D1-D5 groups, respectively; and 0.00 (0) each for the D6-D7 groups. The group-wise mean (standard deviations) R values for the MCs were as follows: D1-D3: 100.00 (0) each, and D4-D7: 99.00 (1.34), 77.10 (3.28), 74.20 (3.61), and 52.20 (4.09), respectively. The one-way ANOVA revealed significant differences between the seven groups in the MCIs, MLIs, and MCs (p<0.05). Conclusions: Our results recommended preparation depths of up to 0.3-0.5-0.7 mm (MCIs), 0.1-0.3-0.5 mm (MLIs), and 0.4-0.6-0.8 mm (MCs) to facilitate complete intraenamel preparation. Moreover, 50% enamel was preserved at preparation depths of 0.5-0.7-0.9 mm (MCIs), 0.3-0.5-0.7 mm (MLIs), and 0.7-0.9-1.1 mm (MCs).

AbstractThis study aimed to investigate the artifacts induced by crowns composed of different materials with prepared teeth and titanium implants. Resin, metal-ceramic, ceramic and zirconia crowns were fabricated and placed onto the prepared teeth on a human cadaver head or titanium implants with prosthesis abutments on a dry human mandible. The samples were scanned on a 1.5 T MRI apparatus, and artifact areas were defined as the signal intensity and signal loss adjacent to the prosthesis and measured by a threshold tool with ImageJ2x. Data were analyzed using SPSS 22.0. Resin, ceramic, zirconia, and precious metal-ceramic crowns barely produced artifacts on the cadaver skull (p > 0.999). By contrast, pure Ti and nonprecious metal-ceramic crowns created significant artifacts (p < 0.001). The average artifacts reduction of double Au-Pt and Ag-Pd metal-ceramic crowns combined with titanium implants and abutments was 79.49 mm2 (p < 0.001) and 74.17 mm2 (p < 0.001) respectively, while artifact areas were increased in double Co-Cr and Ni–Cr metal-ceramic crowns by 150.10 mm2 (p < 0.001) and 175.50 mm2 (p < 0.001) respectively. Zirconia, ceramic and precious metal-ceramic crowns induce less MRI artifacts after tooth preparation while precious metal-ceramic crowns alleviate artifacts in combination with titanium implants.

Introduction. Hall’s technique preformed metal crown (HTPMC) has been used widely by pediatric dentists in developed countries as a new approach for managing decayed primary molars without local anesthesia, caries removal, and tooth preparation. Currently, inadequate information is available regarding the implementation of this technique (HTPMC) in Malaysia. This study is aimed at evaluating the implementation of HTPMC by Malaysia’s pediatric dentists and identify the co-occurrence frequencies of the HTPMC implementation with the respondents’ demographic profile. Materials and Methods. A cross-sectional questionnaire-based research was conducted among 65 pediatric dentists in Malaysia. Online questionnaires were distributed to the pediatric dentists employed at public hospitals (MOH) and universities in Malaysia. Result. It was found that over half of the respondents (65.6%) employed HTPMC. The analysis of the co-occurrence network frequency revealed that a high frequency of female pediatric dentists who were within the age group of 31-40 years old had fulfilled their postgraduation overseas and was employed in the university mainly applied HTPMC. Conclusion. The application of HTPMC among respondent pediatric dentists in Malaysia was high. However, most respondents considered HTPMC a treatment option only to manage carious primary molar rather than a treatment of choice.

Background: Regarding the new dental licensing regulations in Germany (AOZ), this study evaluated the effectiveness of two different digital tooth preparation validation systems in comparison to traditional faculty feedback. Methods: Participants were randomly divided into groups: Faculty Feedback (FF: n = 33), PrepCheck® (PC: n = 32) and Dental Teacher™ (n = 32). Students had the task to prepare tooth 16 for a retentive full-cast crown. Preparations could be repeated as often as desired. Feedback was provided either by faculty staff or by digital validation systems only. Exams were conducted and graded by two independent and experienced examiners. A survey was performed to evaluate the assessment concepts. Results: No statistical difference in examination performance between groups could be observed. Nevertheless, the survey showed participants preferred consulting the faculty staff rather than the digital validation systems. Students preferred practising with DT rather than with PC. Conclusions: Although both classical and digital methods showed comparable results regarding the preparation examination performance, direct feedback by the faculty staff is still appreciated by the students. A combination of both methods is mandatory since demonstration and advice by the teacher is needed. However, digital tooth preparation validation systems are predestined for free practice sessions, providing self-assessment.

Fixed partial dentures (FPDs) made of metal ceramic are a popular treatment option for missing teeth. Studies have shown that posterior abutments of FPDs had a higher pulp survival rate than anterior abutments. Postoperative sensitivity following the cementation of a fixed prosthesis is a frequent symptom, especially when the abutments include important pulp. Dentinal hypersensitivity affects between 4 and 74 percent of people. Females are found to have a somewhat greater DH incidence than males. While DH can affect individuals of any age, there have been several theories on what causes abutment sensitivity after tooth preparation and cementation. In this review we included some of it. Also, we discussed methods of management of postoperative sensitivity and Management of fractured abutment screw.

Abstract The aim of this study was to evaluate the survival of laminate veneers constructed using a recent polymer-infiltrated ceramic network material following the aesthetic pre-evaluative temporary (APT) technique of tooth preparation in comparison to traditional technique. Six patients received 54 laminate veneers. They were divided into two equal groups (n=27) according to the technique of tooth preparation: group T: traditional technique and group A: aesthetic pre-evaluative temporary technique. VITA ENAMIC material was used for CAD/CAM construction of laminate veneers. Cementation was performed using a light cured resin cement. The laminate veneers were evaluated at baseline, after 3, 6 and 12 months according to the modified United States Public Health Service (USPHS) criteriea. The data was collected, tabulated and statistically analyzed. Secondary caries, endodontic complications, cracks and loss of retention were not noted in any laminate veneer. Extensive fractures were not detected in both study groups through the study period. There was a statistically significant decrease of color match criteria between the two groups at the 6 and 12 months recalls. Based on this study, both preparation techniques resulted in successful clinical performance. After 12 months, all the veneers in both groups showed no post-operative sensitivity and all patients were highly satisfied regarding their veneers. However, there was a deterioration in color match criteria through the study period in both study groups.
