Adequate Use of Economic Evaluations Based on the Endpoints of a Registrative Clinical Trial: Cost-Minimization Analysis and the Return of the Living Dead

Diabetes is increasingly widespread in the Russian Federation. It is a severe chronic disease that both independently reduces quality of life and causes exacerbation of other pathologies. This makes evaluation of quality of diabetes management and understanding of clinical and economic properties of various technologies used to screen blood glucose levels, including portable blood glucose meters. Previously there already were clinical-economic evaluations of blood glucose meters within context of Russian Federation but they have not included modern high-precision equipment, thus ensuring attractiveness of current research effort into HTA of modern blood glucose meters that accounts for real life hospital practice. Goal. To assess the clinic-economic properties of modern blood glucose portable measurement systems of OneTouch family and the real-world practics of their use in Russian healthcare facilities. Methodology. The research was performed from the perspective of the Russian healthcare system. Target population was represented by a virtual cohort of 100 patients. Only direct costs were accounted for, specifically all costs caused by consumables involved in operating multifunctional laboratory complexes and all consumables involved in operating portable glucose meters. Employee salaries were also included (accounting for number of personnel involved in different procedures as based on a real clinical practice survey performed as part of this research). Since it is known that modern, standard-compliant portable glucose meters have an accuracy that is more than sufficient for routine screening, cost minimization analysis was used for assessment of economic effects when comparing different approaches to organizing glucose level screening. Research results. The survey during this research effort has indicated that not all medical facilities utilize “tandem usage” of multifunction laboratory complexes and portable glucose meters, however, such “tandem use” is the most widespread approach to organizing blood glucose screening. Cost minimization analysis has demonstrated that “tandem usage” is also the most economically attractive approach and results in cost savings for the medical facility. Conclusions. Current research effort which included cost analysis and cost minimization analysis has demonstrated that “tandem usage” approach which involves combined use of both multifunction laboratory complexes and portable glucose meter systems has substantial economic advantage (over 60 % for analysis with 5 year time horizon). Multiple sensitivity analyses have confirmed robustness of this result. This research further establishes the importance of communicating advantages of modern portable glucose meters to healthcare organizers since this technology is not only clinically rational but also economically optimal.

SUMMARY Objectives: The aim of this study was to assess four post-retained restorative strategies for endodontically treated teeth using cost-minimization analysis. Methods and Materials: The cost-minimization analysis was based on primary data from a randomized clinical trial and followed the Consolidated Health Economic Evaluation Reporting Standards (CHEERS) guidelines. Two hundred twenty-five teeth (141 patients) restored using four strategies—teeth with ferrules+ restored with either glass fiber posts or direct composite or crowns, and teeth without ferrules- restored with either glass fiber or cast metal posts with crowns—were evaluated annually between 2009 and 2018. Initial costs and incremental costs per year were calculated. Survival curves were created using the Kaplan-Meier method and log-rank test. Kruskal-Wallis analysis was followed by Dunn’s test, which was used to compare restorative treatments, with a significance level of 5%. Results: Initial costs were greater for cast metal posts without crowns (US$153.14). Glass fiber posts with composite (US$27.11) were least costly; the most failures occurred in this group, but they were primarily repairable restoration fractures. The number of extractions, and thus cost, was greater for glass fiber posts with crowns. The mean annual cost was significantly lower for teeth restored with composite (p<0.001). Ferrule presence did not significantly impact annual costs. Conclusions: The use of glass fiber posts and direct composite incurred significantly lower annual costs than did other alternatives involving crowns or metal posts.

Background: Asthma and Chronic Obstructive Pulmonary Diseases (COPD) are well known respiratory diseases affecting millions of peoples in India. In the market, various branded generics, as well as generic drugs, are available for their treatment and how much cost will be saved by utilizing generic medicine is still unclear among physicians. Thus, the main aim of the current investigation was to perform cost-minimization analysis of generic versus branded generic (high and low expensive) drugs and branded generic (high expensive) versus branded generic (least expensive) used in the Department of Pulmonary Medicine of Era Medical University, Lucknow for the treatment of asthma and COPD. Methodology: The current index of medical stores (CIMS) was referred for the cost of branded drugs whereas the cost of generic drugs was taken from Jan Aushadi scheme of India 2016. The percentage of cost variation particularly to Asthma and COPD regimens on substituting available generic drugs was calculated using standard formula and costs were presented in Indian Rupees (as of 2019). Results: The maximum cost variation was found between the respules budesonide high expensive branded generic versus least expensive branded generic drugs and generic versus high expensive branded generic. In combination, the maximum cost variation was observed in the montelukast and levocetirizine combination. Conclusion: In conclusion, this study inferred that substituting generic antiasthmatics and COPD drugs can bring potential cost savings in patients.
