Edentulous patients require an adequate rehabilitation so that the alveolar ridge in the interforaminal region be restored for subsequent implant-supported overdentures. The ultimate goal of distraction is to reconstruct the alveolar ridge to a suitable height and width compatible with Atwood class 2 in an appropriate direction allowing the sagittal interalveolar relation to be normally restored. Methods. A 65-year-old man presented with a grade 4 Mandibular atrophy as per Atwood classification which resulted in unsatisfactory treatment with full dentures. Endo-Distractor Krenkel® device was used for anterior mandibular vertical distraction osteogenesis. Four mandibular implants (ITI Straumann, Basel, Switzerland) measuring diameter Ø = 4.1  mm and length L = 16   mm were inserted after the required retention period. Standardized prosthetic treatment was completed with titanium bar retained over dentures. Results. A distraction of 11 mm was achieved within 18 days followed by a retention period of 4 months. No signs of infection nor distractor anchorage loosening were detected, and minimal lingual tilting has occurred. Conclusion. Distraction is possible on severely atrophic mandibles. The quality of bone reconstruction is satisfactory for both functional and esthetic results.

Objective: The objective of this study was to evaluate the biomechanical effect of mandibular corpus distraction osteogenesis with different orientations and rates. Materials and Methods: A three-dimensional model of the mandible was created. The vertical surgical cut was made, the force was applied horizontally in a bidirectional manner within two orientations: parallel to the occlusal plane and parallel to the inferior border of the mandible with three rates (0.5mm, 1mm and 1.5mm). Results: The maximum values for von Mises stress when the force was applied parallel to the inferior border of the mandible with all three rates were smaller than those with force direction parallel to the occlusal plane. The displacement in all three directions x, y, and z were not parallel and prominent in the anterior part of the mandible, while the movement at the posterior part is negligible, x and z displacement were bigger when force was applied parallel to the inferior border of the mandible, z displacement was more prominent than x and y displacement, both directions produced upward rotation of the mandible, this rotation was more noticeable when the force was applied parallel to the inferior border of the mandible. Conclusions: A vertical cut can be used in the patient with a long anterior face. This site of distraction achieves more lengthening of mandible than expansion.

The palatal forms in 20 infants with a complete unilateral cleft lip, and palate (12 with a Hotz plate and 8 without, selected at random) were studied from birth until 18 months of age. Using techniques developed previously, the degree of curvature In the palate and the magnitude of migration of the maxillary segments were measured three-dimensionally. Furthermore, using a newly developed method to approximate a set of the points on the alveolar ridge to a circle in a plane, the form of the alveolar arch was evaluated. Results from the group with a Hotz plate revealed that the plate possesses four effects not seen in the group without a Hotz plate. The size of the palate was larger, and the sagittal gap between the two segments of the maxilla was smaller. These results suggest that the appliance could stimulate the growth of the segments and could prevent collapse of the maxillary arch from the force of lip closure. Third, the steepness of the segments toward the nasal cavity was smaller, possibly occurring because the appliance prevents tongue intrusion into the cleft. Fourth, the magnitude of migration of the lesser segment toward the cleft edge of the major segment was larger. This result suggests that the appliance could guide the growth of the maxillary segments to narrow the cleft width until 18 months of age.

Objective: In this article, we explored the microscopic structure and composition of the decellular-ized cancellous bone matrix of the calf, and established the animal model of Beagle dog extraction. By applying different bone substitute materials in the extraction of teeth, we observed the new collagen-rich in the preservation of the site after tooth extraction. The protein bone matrix maintained the three-dimensional shape of the alveolar ridge compared to other biological materials. Methods: The microstructure of the new collagen-rich bone matrix was observed by scanning electron microscopy. The porous structure, porosity and distribution of collagen fibers were observed. XRD and infrared spectroscopy were used to further detect the inorganic and organic components in the new collagen-rich bone matrix. The premolar extraction and site preservation model of Beagle dogs were constructed. The changes of collagen-rich bone matrix, Bio-oss bone powder, CGF filling and blank control alveolar ridge volume were compared by CBCT. HE staining was used to observe and compare new bone formation, bone remodeling and bone resorption between groups, and to observe the formation of blood vessels, osteogenic mineralization, trabecular bone formation and inflammatory response in different periods. Results: (1) The acellular bone matrix of bovine cancellous bone completely removes the immunogenicity of the cells and has good histocompat-ibility; the pore diameter and porosity closest to the physiological structure, the main component is hydroxyapatite and collagen. (2) Site preservation can reduce the absorption of alveolar ridge following tooth extraction, preserve sufficient bone mass for alveolar fossa, and retain a good width of attachment, which provides further protection for implant surgery. (3) In the post-extraction site preservation, the CGF group has better bone composition than the BABM group and Bio-oss bone powder. Conclusion: Bovine Acellular Cancellous Bone Matrix is a new type of biological bone matrix. The main components are collagen and hydroxyapatite, which can promote bone formation in the extraction socket.
