Linear Lichen Planopilaris of the Trunk: First Report of a Case

Background: Lichen planopilaris (LPP) is believed to be a follicular variant of lichen planus that affects pilosebaceous units, mainly of the scalp. An extremely rare variant of LPP is a linear form, which follows the lines of Blaschko. Of the five previously documented cases of linear LPP, all were limited to the face. Objective: We report the case of a 34-year-old male who presented with a nonpruritic eruption on the trunk consisting of erythematous, keratotic, folliculocentric papules following Blaschko's lines. Results: Biopsy revealed lichenoid and interface dermatitis involving the basilar epidermis and hair follicles, as well as apoptotic keratinocytes, consistent with LPP. Conclusion: This represents the first documented case of LPP, following the Blaschko's lines, in a nonfacial distribution.

Background: Primary cutaneous anaplastic large cell lymphoma (PC-ALCL) is a T-cell lymphoma that presents in the skin and consists of CD30+ anaplastic large lymphocytes with abundant cytoplasm and pleomorphic nuclei. In addition to the classic variant, several other histologic patterns of ALCL have been identified, among them the lymphohistiocytic variant. Objective and Methods: We describe a case of the lymphohistiocytic variant of PC-ALCL with a marked follicular involvement. The lesion appeared as a single nodule in the face that completely regressed following biopsy. A diffuse infiltration of lymphocytes and macrophages was seen in the dermis associated with hypertrophied hair follicles, follicular mucinosis, and marked folliculotropism, leading to follicular disruption. Cohesive groups of CD30+ large, atypical lymphocytes with a high proliferative index were seen focally. The patient was followed up for 41 months, during which time no relapses occurred. Conclusions: This case shows that PC-ALCL may present with different histologic features, including a follicular variant, that may mimic both benign and malignant conditions.

The follicular variant of lichen planus is also known as lichen planopilaris. it is an inflammatory, primary cicatricial alopecia which accounts for 30-40% of scarring alopecia. It usually occurs between 30 to 70 years of age group with a female predominance of varying ratio ranging between 1.8:1 and 9:1. LPP is usually an insidious process evolving over several years. It usually presents as irregular patchy hair loss with loss of follicular ostia and perifollicular erythema and perifollicular scales are typically present at the periphery of active lesions. So, hereby reporting this case for its rarity in occurrence in male patient.

Lichen planus is a T-cell mediated autoimmune disorder affecting the skin and mucous membranes. Ulcerating lichen planus is uncommon mostly on oral and genital mucosa but not skin. Lichen planopilaris, however, is a subtype of lichen planus affection hair follicles and leading to permanent scarring alopecia. We report a case of lichen planopilaris of the scalp with multiple alopecic patches ulceration – a hitherto unreported clinical feature. The patient was treated surgically, and the defect could be closed by combined tissue advancement and extension.

Pigmented lichen planus is a rare variant of lichen planus that is characterized by oval and round shape dark brown macules with indistinct border. Linear pigmented lichen planus is a clinical pattern of planus disease that presents unilaterally along one or more lines of blaschko. We report a 30–year-old female patient with Linear pigmented lichen planus along the lines of blaschko with mucosal involvement. ABSTRACT The trichoepithelioma is a benign cutaneous neoplasm which is derived from hair follicles. It is common in the face, but there are only three reports of the solitary occurrence on the nose. It is often not recognized because of its rarity, controversial classification, origin and biological potential. The objective of this paper is to present a case of solitary trichoepithelioma on the nose, histopathological examination and treatment. It should be considered as a differential diagnosis of a solitary lesion of nose which is confused with basal cell carcinoma. The confirmation by histopathological examination is essential.
