<p>Action research is a critical reflective process that involves spirals of cycles if of planning, acting, reflecting/evaluating and replanning the next cycle. This action research inquiry explores communication and nursing practice in an effort to improve practice and enhance patient care. Implications of this study indicate that action research is a method that works, and it is a satisfying way of challenging and changing nursing practice.  Using ear syringing as a procedure, in the general practice setting and at two separate surgeries, another Practice Nurse and I co-researched this study during working hours. 12 people consented to participate in the research that involved the audiotaping of each ear syringing interaction. Following each transcription of the recording, my co-researcher and I read our own and then each other's transcripts, and listened to the recordings. We then met to discuss and reflect on our findings and to plan the next cycle.  Throughout the process, my co researcher and I found a number of areas of practice we could change or enhance. Changes included the use of technical language such as "contraindications" and "auditory meatus", the side effects of syringing, improvements in communicating situations where ear syringing is not recommended and the options available, and post procedure information. These changes became a significant challenge and areas for improvement when both my co-researcher and I forgot the changes, thus repeating previous errors and omissions. This factor highlighted the need to practise any changes prior to interactions, and to have a cue card on hand to facilitate recollection and to cement improvements into practice.  Although time constraints limited this inquiry to three cycles, at the final meeting we agreed to continue the reflective process we had begun to explore our practice.</p>

Im Jahr 2008 wurde in der kinderkardiologischen Abteilung des Universitätsklinikums Freiburg i. Br. das patientenorientierte Konzept Advanced Nursing Practice (ANP) eingeführt, um den Bedürfnissen der dort behandelten, meist chronisch kranken Patienten zu begegnen. Ziel von ANP in diesem Bereich ist es, den Patienten und deren Eltern bzw. Familien eine erweiterte und vertiefte Betreuung und Pflege zu bieten. Die Advanced Practice Nurse (APN), eine Pflegende mit einem Masterabschluss in Pflegewissenschaft, arbeitete dabei in der direkten Betreuung von Patienten und Eltern. Im Rahmen von leitfadengestützten Fokusgesprächen sammelte die APN Themen für mögliche unterstützende Interventionen. Aus dieser Sammlung heraus erstellte sie Schulungs- und Beratungsprogramme wie die Medikamentenberatung vor der Entlassung und die Schulung der Eltern zum Erkennen von Herzinsuffizienzzeichen bei ihrem Kind. Die Beratungsprogramme sollen den Patienten und Eltern mehr Sicherheit geben und die Eltern in ihrer Dependenzpflegekompetenz unterstützen. Die bisherige Rückmeldung von Seiten der Patienten, Eltern und Mitarbeiter zur Umsetzung des ANP-Konzepts ist positiv, eine Evaluation steht noch aus. Es zeigte sich, dass die Implementierung und Umsetzung eines solchen Konzepts Zeit braucht und dass Rahmenbedingungen wie beispielsweise ein fachlich versiertes Pflegeteam vorhanden sein müssen. Um diese Rahmenbedingungen zu schaffen, musste die APN vor allem in den ersten Monaten, aber auch jetzt noch die fachliche Entwicklung der Pflegenden in ihren Fokus mit aufnehmen.

Abstract: This paper reports reflections of one of the courses I took in this semester namely Methodologies of Educational Research. To construct this paper, I used qualitative biographical-reflective method which is aligned with interpretive paradigm of narrative inquiry research methodology to describe my own experience in the learning process. The data I used were the postings from Zuvio mobile application designed for advising learning process with online platform and personal experience from reflective process of the course. I analyzed the data with three-stage qualitative data analysis: data reduction, data display, and conclusion. The findings report that the classroom undergone with classroom action research has successfully overcome the difficulty faced by professors and students at international PhD programs. The students were found to perceive positive attitudes towards the carefully selected instructions design of action research with Flipped-Jigsaw approach. The implications were also proposed. Abstrak: Artikel ini melaporkan refleksi dari salah satu mata kuliah yang saya ambil pada semester ini yaitu Metodologi Penelitian Pendidikan. Untuk menyusun makalah ini, saya menggunakan metode kualitatif biografis-reflektif yang selaras dengan paradigma interpretatif metodologi penelitian inkuiri naratif untuk menggambarkan pengalaman saya sendiri dalam proses pembelajaran. Data yang saya gunakan adalah postingan dari aplikasi mobile Zuvio yang dirancang untuk proses pembelajaran dengan platform online dan pengalaman pribadi dari proses reflektif kursus. Saya menganalisis data dengan tiga tahap analisis data kualitatif: reduksi data, penyajian data, dan penarikan kesimpulan. Temuan penelitian ini menunjukan bahwa kelas yang dilakukan dengan penelitian tindakan kelas telah berhasil mengatasi kesulitan yang dihadapi oleh profesor dan mahasiswa di program PhD internasional. Para siswa merasakan sikap positif terhadap desain instruksi penelitian tindakan yang dipilih dengan cermat dengan pendekatan Flipped-Jigsaw. Implikasinya juga diusulkan pada artikel ini.
