Chronic pain in the elderly usually has negative impacts on physical and psychological status, therefore, early diagnosis and treatment should be performed. The principle of chronic pain management is assessment of pathophysiology which leads to different choices of treatment. Furthermore, chronic pain management in the elderly should be combined nonpharmacological such as cognitive-behavioral therapy and pharmacological treatment in order to increase efficacy of pain control. Pharmacological management for chronic pain is composed of 3 categories including nonopioid analgesics, opioid analgesics, and adjuvant medications. The strategies of pharmacological treatment in the elderly are consideration of start with a low dose and slow titration. Moreover, physicians and pharmacists should be aware of drug-drug interactions, drug-disease interactions, as well as adverse drug reactions and events during treatment.

The decreasing of biological function in the aging process affecting a damage in organ tissue in the elderly, so they are easy to be susceptible of diseases including chronic pain. Pain management in the elderly with osteoarthritis in the community in large mount use pain relievers, and rarely use non-pharmacological therapy, even though the use of non-pharmacological pain management has a smaller risk of drug side effects. The solution needed to be done is Assistance to elderly in Independent Nursing Services with Chronic Pain Problems. The implementation method is providing health education to elderly families, mentoring chronic pain management independently, assisting in developing an integrated healthcare center program for the elderly on chronic pain management and conducting training for elderly health cadres. The results of the implementation of mentoring for elderly were carried out in four activities including health counseling for the elderly and family, assistancing in managing chronic pain independently, optimizing the elderly integrated healthcare center program on pain management and training for elderly health cadres. Assistancing for the elderly in independent nursing services as part of human resource development efforts in improving the handling of problems that occur in the elderly, one of which is chronic pain in the elderly, namely with independent nursing services that can be done at home, so that it becomes part of promotive and preventive efforts

BACKGROUND: As the elderly population increases dramatically, the chronic age-related disease should be noted. In the elderly, chronic pain is a common health problem. METHODS: This search was performed in 3 databases (PubMed, Google Scholar, Embase). We have reviewed articles related to pain management in the elderly. RESULT: The prevalence of pain in people aged above 60 is twice that in younger people. Pain is estimated to be 45-85 per cent in the elderly. Pain is not a part of the ageing process, but many older people can experience it. Perception of pain can be affected by environmental, emotional, cultural and cognitive factors. Pain in the elderly often remains untreated and misdiagnosed. CONCLUSION: Pain management in elderly needs different approach because of unreported pain in this population, and usually they have multiple problems and comorbidities that complicate evaluation and treatment.
