Piriformisa syndrome is a subgroup of the deep gluteal syndrome, an important differential diagnosis of sciatica. Piriformis is a short external rotator muscle of the hip joint passing close to the sciatic nerve as it passes through the great sciatic foramen. Compression causes numbness, ache or tingling in the buttocks, posterolateral aspect of the leg and foot. The causes of sciatic nerve entrapment in the deep gluteal syndrome are best shown by endoscopic exploration. The frequency of anatomical variants in normal subjects however, should caution that such anomalies are not necessarily the cause of symptoms.

Abdominal cutaneous nerve entrapment is a rarely diagnosed condition that leads to intense neuropathic pain in the anterolateral wall of the abdomen. Generally, it is triggered by some factor implied in the increase of the pressure on the nerve in its passage by the abdominal wall. Its most important differential diagnosis is pain of visceral origin. We present a case in which the clinical findings confirmed on ultrasound and other imaging tests established the diagnosis and in which the noninvasive treatment was effective.

AbstractPiriformis syndrome is a rare cause of sciatica, which results in low backache due to sciatic nerve compression. This syndrome is associated with abnormalities in the piriformis muscle, which cause sciatic nerve entrapment, like anatomical variations, muscle hypertrophy, and inflammation. It can also result from the abnormal course of sciatic nerve itself through normal piriformis muscle. Piriformis syndrome due to pyomyositis of the piriformis muscle is extremely rare and only 23 cases are reported in literature. Herein, we report one such rare case of a patient, with pyomyositis of piriformis muscle, who presented with piriformis syndrome.
