The posterior maxilla is indubitably the site reported with highest failures in implant literature so far as the bone availability beneath the sinus poses a major challenge for surgeons. Sinus lift both by direct and various indirect accesses were hitherto utilized to counter the compromising situation. But the risk of implant failure if the implant was simultaneously placed besides the parasthesia, perforation and morbidity were experienced more with more the cases documented. The osseodensification(OD), a relatively new technique provides a minimally invasive approach along with feasibility where the above-mentioned other modalities are contraindicated. The present article presents the novel case of indirect sinus lift with OD concept and simultaneous implant placement using an adjunct PRF (Platelet-rich fibrin).

Maxillary sinus floor elevation via a lateral approach is a predictable technique to increase bone volume of the edentulous posterior maxilla and consequently for dental implants placement. The sinus floor is elevated and it can be augmented with either autologous or xenogeneic bone grafts following an opening bone window created on the facial buccal wall. Maxillary septa are walls of cortical bone within the maxillary sinus. The septa shape has been described as an inverted gothic arch arising from the inferior or lateral walls of the sinus and may even divide the sinus into two or more cavities. Some authors have reported a higher prevalence of septa in atrophic edentulous areas than in nonatrophic ones. Radiographic identification of these structures is important in order to perform the right design of the lateral window during sinus lift. Aim of this investigation is to highlight the correct steps for doing sinus lift surgery in presence of those anatomic variations. Clinicians should always perform clinical and radiographic diagnosis in order to avoid complications related to the sinus lift surgery.

The sinus lift is a procedure that is used frequently in oral surgery, different techniques exist to perform this procedure approaches, which allow the surgeon to give a sufficient bone height for planning a rehabilitative treatment with implant at the required site. In this case the unilateral lifting of the maxillary sinus floor by buccal bone window, with subsequent implant placement and lyophilized human bone in a single surgical procedure.

ABSTRACTDue to the angulation of maxillary bone as the maxillary bone resorption occurs from a buccal to palatal direction, bucco-lingually tilted implant placement is more likely especially in the posterior maxilla. The aim of this paper was to present a case restored by using a maxillary hybrid denture using Cresco method. A 59-year-old woman with an edentulous maxilla and five missing mandibular teeth was referred for implant treatment. Six months after sinus lift operation, four mandibular implants and six maxillary implants were placed. Due to the bucco-lingual tilt of two implants, the Cresco method was chosen to fabricate the metal framework for the maxillary hybrid denture that was delivered four months after implant placement. The main advantages of the Cresco method are a perfectly passive framework fit and corrected screw access holes. All ten implants placed were considered successful at 1-year recall, and no screw access holes were seen in the buccal part of the maxillary acrylic teeth.It was concluded that such screw-retained maxillary hybrid denture using the Cresco method met the esthetic expectations of a highly demanding patient. Maxillary sinus is the first paranasal sinuses to develop, which is pyramidal in shape and it completes developing around 20 years of age with the eruption of the maxillary third molars. Pneumatization of the maxillary sinus occurs with time. Tooth loss may lead to loss of bone density, atrophy of bone and further pneumatization of the maxillary sinus leading to insufficient quality and quantity of bone for placing implants. Despite lots of literature and research being done, there is no clarity in obtaining consensus regarding the techniques and materials used in maxillary sinus lift procedures. This article reviews the various techniques and bone graft materials used in maxillary sinus lift procedures. The techniques include lateral window approach, hydraulic sinus lift technique, Piezoelectric Surgery technique, Transcrestal Approach, Osteotome Technique, and Balloon elevation technique. It can be concluded that the balloon antral elevation technique and Hydraulic Sinus Lift technique are more efficient techniques for maxillary sinus lift procedures. These techniques are known to result inless perforations, less chair-side time, comparatively easier, and the need for elaborate instrumentation is minimized. It can also markedly increase the success rates of implants in contrast to the conventional techniques which pose greater risks to the patient, more soft tissue trauma, more chair-side time and can expose patients to infections.

Sinus lift operations have become a prerequisite for implantologists in the rehabilitation of atrophic posterior maxilla. Alloplasts and xenografts are the recent innovation in the world of grafting materials and have evolved in such a way as to compensate the need for autografts to be used in conjunction. In order to perfect the grafting materials, histomorphometry is the most often tool used to study the success of the augmentation. This article has tried to commemorate the importance of histomorphometry in maxillary antroplasty and also the success rate of β-tricalciumphosphate and deproteinized bovine bone as an augmentation material in maxillary augmentation.
