Ovarian Ectopic Pregnancy: A Rare Case Report

ABSTRACT Ovarian pregnancy is the most common type of non-tubal ectopic pregnancy. Ovarian ectopic pregnancy incidence after natural conception ranges from 1 in 2000 to 1 in 60,000 deliveries and accounts for 3% of all ectopic pregnancies. Here, we report a rare case of ruptured ectopic pregnancy. A 30 years old, G2P1+0L1 was admitted with amenorrhea of 1½ months and severe pain abdomen. Self urinary pregnancy test (UPT) was positive. Ultrasonography (USG) revealed it sided adnexal mass. Emergency laparotomy was done and a diagnosis of ovarian ectopic pregnancy was made. This case presenting a 30-year-old lady, a case of G4A1E2 who presented with history of 1 month of amenorrhoea, followed by bleeding per vaginum 1 week back for 3 days. Presented to hospital with spotting PV and pain abdomen for 2 days. UPT done at home was positive. USG revealed right ovarian complex haemorrhagic cyst and left adnexal ectopic pregnancy near left ovarian fimbriae. Conservative management by Methotrexate and Folinic acid regimen was tried but as there were no signs of resolution on serial ultrasonography reports and serial human chorionic gonadotropin (hCG) levels did not fall as expected, hence decision of laparoscopy/laparotomy was made and salpingectomy was done. Later patient was followed up with serial hCG levels.

Abdominal ectopic pregnancy accounts for only 1.3% of all ectopic pregnancies and occurs when a fertilised ovum implants in an extra pelvic peritoneal location. Splenic pregnancy is one of the least common form of abdominal ectopic pregnancy. Early diagnosis is essential as delay in treatment carries significant potential for morbidity and mortality. We present a case of 22-year-old female with 6 weeks amenorrhoea and acute abdomen. Patient underwent emergency exploratory laparotomy with suspicion of ?splenic hemangioma  ?ruptured ectopic pregnancy. Splenectomy was performed. Final diagnosis was confirmed on histopathology showing chorionic villi invading the splenic parenchyma.

Ovarian pregnancy is an uncommon presentation of ectopic gestation and usually, it ends with rupture before the end of the first trimester. Its presentation often is difficult to distinguish from that of tubal ectopic pregnancy and hemorrhagic ovarian cyst. We report a rare primary ruptured ovarian pregnancy in a 26 years lady. Background:Ovarian pregnancy is classified as a rare cause of non tubal pregnancy wherein maximum ends in rupture in early months of pregnancy. Sign and symptoms often mimic tubal rupture. To distinguish between the two based on presenting complaints and ultrasonography findings is difficult. Generally confirmation of ovarian preganancy is done only after histopathological examination due to its similarity in presenting complaints to tubal pregnancy. Medical management has also been tried for unruptured ectopic pregnancy. Case:Presenting a rare case report of ruptured left ovarian pregnancy. pt was 26yrs old multiparous with previous two cesarean 7yrs and 4 yrs back with one MTP kit taken 6 months back with pain in left iliac fossa. USG findings were suggestive of tubal rupture, however during laparotomy ruptured left ovarian pregnancy was diagnosed followed by left oopherectomy later on confirmed by histopathological examination.

The mortality and morbidity rates of non-tubal ectopic pregnancies with abdominal hemorrhaging are 7–8 times higher than those of tubal pregnancies. Diaphragmatic pregnancy is a rare non-tubal ectopic form, causing acute abdominal hemoperitoneum. Here, we present a case of a primary diaphragmatic ectopic pregnancy with hemorrhage that was immediately diagnosed and successfully managed with laparoscopic surgery. Rapid and accurate diagnosis using appropriate imaging modalities is critical for improving the prognosis of a child-bearing woman with an abdominal pregnancy.

Gastroduodenal tuberculosis is uncommon even in parts of world where tuberculosis is endemic and accounts for 2.3 % of abdominal tuberculosis. We present here a case of duodenal tuberculosis who presented with vomiting, pain abdomen and weight loss. Duodenoscopy revealed mucosal ulceration and nodularity with marked enlarged and erythematous surrounding folds. Histopathology of duodenum showed features of tuberculosis.
