Soft tissue augmentation using autogenous gingival grafts is a procedure often used in implantology and periodontal surgery. The technique of connective tissue grafting refers to the placement of connective tissue under a flap of partial thickness, which aims to induce the formation of keratinized tissue. It is indicated for partially or totally edentulous patients to increase the areas with the absence or reduction of the height of the keratinized tissue, as well as to increase the volume of soft tissue. The structural differences between the implant / bone tissue interface and the natural tooth / bone tissue make dental implants more susceptible to the development of inflammatory processes and bone loss. For these reasons, the intangibility of the seal around the implants is one of the primary objectives in its success over a long period of time. The function of the peri–implant seal is to “maintain internal homeostasis in response to external environmental challenges.”

Introduction The presence of healthy attached tissue at the tooth and implant soft tissue interface correlates with long-term success and stability in function and esthetics and requires conversion of unfavorable soft tissue traits to more favorable ones. Several soft tissue grafting procedures have been developed to improve both the volume of keratinized tissue and the soft tissue contour around implants and the tooth. Objectives This study therefore aims at determining the knowledge , attitude , and perception of Periodontists and implantologists in private practice towards soft tissue grafting around implants and teeth . Methodology Approximately 100 periodontists and dental practioners from Mumbai were provided with a questionnaire to evaluate their knowledge about soft tissue grafting around implants and teeth .The questionnaire consists questions regarding different techniques involved in soft tissue grafting , the materials used for it , the complications faced during the procedure and the preferred method . Results: The results show that very few practioners are practicing soft tissue grafting around natural teeth and implants in their private practice. Conclusion : Thus, keeping in mind the results of the study it is essential to create more awareness about soft tissue augmentation and its importance and also conduct programmes to train dentists for the same. Failure of a natural tooth may not permit placement of an implant at the time of extraction due to insufficiency in available bone to house the implant. Reconstruction of the extraction socket frequently involves both hard and soft tissue augmentation to provide a site that can house the implant and ridge contours that mimic the adjacent natural anatomy. The modified IVAN technique achieves those goals and may be used in both delayed and immediate placement situations. The technique will be discussed as well as long-term follow-up on 20 cases treated by the author.

A anatomia do tecido mole peri-implantar é semelhante à do dente natural, uma vez que apresenta epitélio juncional e tecido conjuntivo. O selamento peri-implantar é estabelecido pela mucosa ceratinizada que previne a migração apical da margem gengival, mantendo a arquitetura tecidual e estética vermelha ao redor dos implantes osseointegráveis. O presente trabalho objetivou relatar um caso clínico de reconstrução de tecido mole previamente a cirurgia óssea reconstrutiva para a instalação de implante osseointegrável. Após a reconstrução do tecido mole observou-se que houve um satisfatório ganho de altura e espessura tecidual. Background/Aim. Filling a bone defect with bone substitution materials is a therapy of choice, but the infiltration of connective tissue from the mucoperiostal flap may compromise a healing of bone substitutions with bony wall defects. Application of membrane as a barrier is indicated as a solution to this problem. The aim of this study was to show a pathohistological view of bone regeneration and the significance of human resorbable demineralized membrane (HRDM), 200 ? thick in bone regeneration regarding mandibular defects in an experiment on dogs. Methods. The experiment was performed on six dogs. Bone defects were created in all six dogs on the right side of the mandible after the elevation of the mucoperiostal flap. One defect was filled with human deproteinised bone (HDB), and in between HDB and soft tissue RHDM of 200 ? thick was placed. In the second defect, used as a control one, only HDB without RHDM was placed. Two dogs were sacrificed two months after the surgery, another two dogs four months after the surgery and the last two dogs six months after the surgery. After that, samples of bone tissue were taken for histopathological analysis. Results. In all the six dogs with defects treated with HDB and RHDM the level of bone regeneration was much higher in comparison with the control defects without RHDM. Conclusion. Membrane, as a cover of bony defect, is useful and benefits bone regeneration. Bony defects covered with RHDM show better bony healing despite the fact that bone regeneration was not fully complete for as long as six months after the RHDM implantation.

Following tooth extraction, resorption of the buccal wall of the socket will occur; this will be true for both the maxilla and the mandible. Where the extraction site is surrounded by natural dentition, the loss of the buccal alveolar plate can degrade the visual aesthetics of an implant-supported prosthetic rehabilitation. To aid the harmonization of the hard and soft tissue morphology, both hard and soft tissue augmentation can be carried out either consecutively with an extraction/immediate implant placement or prior to an implant placement in the delayed scenario. The contemporary method of increasing soft tissue volume is to use the Subepithelial Connective Tissue (auto) Graft (the SCTG). The graft requires fixation, otherwise it can be extruded from the recipient site. This article presents a novel suturing technique which can confidently secure the SCTG, thus resisting its dislodgement.
