Introduction: Stroke is considered a relatively common disease in developing societies. This disease is one of the most important causes of sudden non-cardiac incapacity. On the other hand, the disease can naturally occur in working-age and drivers. Therefore evaluation of the fitness for work in a driver with a history of stroke can be a significant challenge for the occupational health physician and road safety. Materials and Methods: In this study, to identify the main indicators and evaluate job fitness in a driver with stroke history, research has been conducted in valid databases such as Embase, UpToDate, PubMed, Medline (Ovid), Scopus, and web of science. Indicators that should be considered in this evaluation include the type and severity of the stroke, the probability of recurrence of the stroke attack, the degree and type of limb defect in the driver following the stroke, etc. Also in this study has been paid to some guidelines for help in deciding how to continue driving. Results: Stroke is the most common acute neurological disease among working-age adults. About a quarter of strokes occur at working age. Cerebrovascular disease can lead to sudden disability. Approximately 50% of people with stroke in developed countries want to continue driving. Conclusion: Things such as the probability of occurrence of stroke and sudden incapacity in the future and comparing this amount with acceptable risk and the amount of limb defect caused by stroke and its interference with driving should be considered. Also, consumption medications and their side effects and other comorbidities should be considered in this job fitness evaluation.

Aplasia cutis congenita (ACC) is a rare condition in which there is congenital focal absence of skin with or without absence of underlying structures such as bone. Consanguinity may play a role. The cause of this condition is unclear and appears to be multifactorial; contributory factors may include teratogens, genes, trauma and compromised blood flow to the skin. Various expressions of Adams Oliver syndrome (AOS) have also been reported which is a rare autosomal dominant congenital disorder characterized by absence of skin and or underlying structure over scalp along with transverse limb defect. It was first described by Adam and Oliver in 1945.

A case of prenatal ultrasound diagnosis of congenital limb defect with combined features fibular aplasia, tibial campomelia, and oligosyndactyly (FATCO syndrome) at 20+4 weeks of gestation is presented. Diagnosis was completely verified macroscopically and with X-ray investigation after termination of the pregnancy at 21+4 weeks of gestation.

AbstractWe report a rare limb defect named as fibular aplasia, tibial campomelia, and oligosyndactyly (FATCO) syndrome in a female monozygotic twin with a normal twin sister, presented with anterior tibia pseudarthrosis, oligosyndactyly, and pes equinovarus. Radiographic examination displayed the absence of left fibulae, anterolateral pseudarthrosis of left tibia, and the absence of some metatarsus and phalangeal bones. Our case report is the first to report that only one of the identical twins was affected by FATCO syndrome, which is a significant finding because the pathogenicity of FATCO syndrome is yet to be identified, and this clinical case may provide a new insight for discovering the etiology of FATCO syndrome.
