ABSTRACT Third or fourth branchial pouch anomaly is rare. It usually presents as an acute inflammatory swelling of the lower neck and frequently involves the thyroid gland. Surgical treatment is essential to prevent recurrence. USG, CT scan and barium esophagography help in the diagnosis.
