Sphenochoanal polyp are rare tumours arising from sphenoid sinus. The main presenting complain is gradually progressing nasal obstruction. Sphenochoanal polyp mimics antrochoanal polyp clinically. To differentiate it from antrochoanal polyp, diagnostic nasal endoscopy, computer tomography and magnetic resonance imaging of the paranasal sinuses are the investigation of choice. Functional endoscopic sinus surgery is the line of treatment.: A 33years old female presented with right nasal obstruction since 2 years. On Diagnostic nasal endoscopy polypoidal mass was seen partially obliterating the right nasal cavity not arising from middle meatus. CT scan showed polypoidal mass obliterating the nasal cavity, choana and Sphenoid sinus. It was completely excised by functional endoscopic surgery. We present this rare case to highlight the use of diagnostic nasal endoscopy and Computed Tomography in the diagnosis and treatment of Sphenochoanal polyps

Introduction The occurrence of isolated spheno-choanal polyps are not very common and can be very well confused with antro-choanal polyps or adenoids in adolescents and children. Appropriate diagnosis and prompt surgical intervention are essential for its removal. Case Report We report one such case of a 16 year old female who presented with complaints nasal obstruction and reduced hearing. Endoscopic polypectomy was done with complete removal of the polyp. Discussion Sphenochoanal Polyp is often misdiagnosed and proper investigations are thus essential for its diagnosis. Surgical Approach is the mainstay form of treatment with complete excision of the polyp.

Sphenochoanal polyp is a rare entity. Isolated polyps arising from mucosa covering the anterior wall of the sphenoid sinus or sphenoethmoidal recess extending till oropharynx is extremely rare. We aim to report a rare case of sphenochoanal polyp with a slender, unusually long stalk which extended till nasopharynx leading to a large bulky polypoidal mass extending till oropharynx. Due to its posterior location, sphenochoanal polyps are often diagnosed late as they can be easily missed in clinical examination. We intend to emphasize the combined use of diagnostic nasal endoscopy and computed tomography imaging in patients presenting with unilateral or bilateral nasal obstruction, snoring and recent onset of sleep disturbances. Treatment includes endoscopic excision of polyp and cauterization of the base of the stalk. Widening of the sphenoid sinus ostium is done when necessary. Early and complete removal and regular follow up is essential to prevent complications.

Choanal polyp arising from the sphenoid sinus and presenting at the choana are rare. These are known as sphenochoanal polyp. We present a case of sphenochoanal polyp and its clinical features and surgical management is discussed. Our aim in this case was to properly delineate the origin of the polyp and differentiate it from other lesions such as the antrochoanal polyp and meningocele, followed by meticulous endoscopic excision of the polyp. Radiological investigations include a CT scan paranasal sinuses or a MRI diagnostic nasal endoscopy will confirm the diagnosis. Once the diagnosis is made surgical removal must be done.
