Introduction The occurrence of isolated spheno-choanal polyps are not very common and can be very well confused with antro-choanal polyps or adenoids in adolescents and children. Appropriate diagnosis and prompt surgical intervention are essential for its removal.   Case Report We report one such case of a 16 year old female who presented with complaints nasal obstruction and reduced hearing. Endoscopic polypectomy was done with complete removal of the polyp.   Discussion Sphenochoanal Polyp is often misdiagnosed and proper investigations are thus essential for its diagnosis. Surgical Approach is the mainstay form of treatment with complete excision of the polyp.

AbstractOsteomas are common benign tumours of the paranasal sinuses, but are usually asymptomatic and found as a coincidental finding during routine radiological investigations. We present a case report of a patient with an isolated osteoma of the middle turbinate presenting with unilateral nasal obstruction which required removal via a lateral rhinotomy approach. The aetiology, pathology and presenting features of these tumours are reviewed and their subsequent management discussed. In particular, adequate pre-operative radiological assessment to determine involvement of the cribriform plate region by computerized axial tomography (CAT Scanning) is emphasized since this may indicate the need for a cranio-facial surgical approach rather than a lateral rhinotomy to achieve complete removal.

Background. Macroprolactinemia is defined as predominance of high molecular weight prolactin forms in the circulation. Although macroprolactin is considered as a biologically inactive molecule, some authorities suggest treatment in symptomatic cases. Gigantomastia is defined as excess breast tissue and most cases in the literature were treated by surgical intervention.Case. A 44-year-old woman was admitted to our clinic with gigantomastia and galactorrhea. The patient had a demand for surgical therapy. In laboratory examination, she had hyperprolactinemia and macroprolactinemia. Pituitary imaging revealed 6 mm microadenoma in right side of the hypophysis. Since she was symptomatic, cabergolin treatment was started. Macroprolactin became negative, breast circumference decreased significantly, and galactorrhea resolved after treatment.Conclusion. Gigantomastia might be the presenting symptom in patients with macroprolactinemia. In these patients medical treatment with cabergoline may be used initially as an alternative to surgical approach.

Introduction. Due to its contents and relations with neighboring regions, the infratemporal fossa has a great clinical significance. Primary tumors of this region, both benign and malignant, are rare, but they do require surgical treatment, which is determined by the size and localization of the tumor. Case Report. The paper presents the case of a 72-year-old female patient who was referred to hospital for paresthesia in the left half of the face after having been found to have a tumor of left infratmeporal fossa by imaging methods. The tumor was completely removed by transfacial-transzygomatic approach and, according to histopathological findings, it was a schwannoma. The postoperative course was without complications and the patient did not have any discomforts at the check-up a month later. Conclusion. In addition to the accurate diagnosis, the success rate of the surgical treatment of tumors depends on the appropriate surgical approach to ensure the complete removal of the tumor while preserving the content of the infratemporal region.

Introduction.Osteoma of the temporal bone is a very uncommon benign tumor of bone. Osteomas may occur in the external auditory canals but are reported to be very rare in the mastoid bone.Case Report.A 36-year-old male presented to our department with a hard swelling behind the right ear diagnosed as osteoma. Complete excision was done through a postauricular approach. Histopathology confirmed the diagnosis of osteoma.Conclusion.Osteomas, although rare, should be considered when dealing with any hard mastoid swelling. Complete removal can be ensured by drilling till the normal cortical bone is reached to avoid recurrence. A cortical mastoidectomy should be done if the mastoid air cell system is involved.

We present a case report of a female patient with complaints of single-sided nasal obstruction. A polypoid structure was seen in the nasopharynx. Histologic examination showed a respiratory epithelial adenomatoid hamartoma – a rare, benign lesion. Therapy consisted of complete excision. In line with previous reports, the lesion did not recur during 13 months of follow up. The clinical and pathological features of this abnormality are discussed.

BACKGROUND Coccydynia refers to debilitating pain in the coccygeal region of the spine. Treatment strategies range from conservative measures (e.g., ergonomic adaptations, physical therapy, nerve block injections) to partial or complete removal of the coccyx (coccygectomy). Because the surgical intervention is situated in a high-pressure location close to the anus, a possible complication is the formation of sacral pressure ulcers and infection at the incision site. OBSERVATIONS In this case report, the authors presented a minimally invasive, fully endoscopic approach to safely perform complete coccygectomy for treatment of refractory posttraumatic coccydynia. LESSONS Although this is a single case report, the authors hope that this novel endoscopic approach may achieve improved wound healing, reduced infection rates, and lower risk of penetration injury to retroperitoneal organs in patients requiring coccygectomy.

Abstract The rationale behind endodontics is to thoroughly disinfect the root canal system, and to provide hermetic seal which in turn ceases the progress of the periapical infection. Most of these periapical infections attributed directly or indirectly to oral microbes. Surgical interventions in larger periapical infection are sometimes accompanied by certain complications. Hence, non-surgical approach can be considered. This article presents non-surgical healing of a periapical lesion of endodontic origin associated with maxillary left central and lateral incisors, observed over a period of 6 months, using calcium hydroxide as an intracanal medicament. Multiple epidermal cysts over the scrotum is a rare condition and requires surgical intervention if infected or cosmetically unacceptable.  Grossly enlarged cysts require complete excision, followed by scrotoplasty.  We are reporting a case of multiple epidermal cysts on the scrotum.  Complete excision of the cysts followed by scrotoplasty was done in this case.

Abstract Osteomas are benign tumours that involve paranasal sinuses more than other parts of body. Frontal sinus is most common while sphenoid is least one. They are usually asymptomatic and diagnosed incidentally on imaging for other reasons. Giant variety of frontoethmoid osteoma is very rare and only few cases are described in literature. Orbital involvement in giant frontoethmoid osteoma is even rarer. Due to threat to vision, this variety is an indication for surgical intervention followed by reconstruction of defect. As these tumours are very large, endoscopic approaches are less indicated and there is no clear consensus which open surgical approach is better. We are reporting such a rare case of giant frontoethmoid osteoma causing proptosis and restriction of eye movement that was treated with open surgical approach.

RESUMOIntrodução: A fratura de pênis trata-se de uma urgência urológica incomum e pode ser definida como trauma peniano fechado que resulta na ruptura da túnica albugínea. A apresentação clínica do quadro é descrita pelo paciente como um “estalido” ocorrido pelo rompimento do corpo cavernoso, acompanhado de dor, detumescência peniana imediata, edema e hematoma. Durante o exame observa-se o desvio peniano para o lado oposto ao da fratura. O diagnóstico da fratura peniana é clínico. O tratamento preconizado seria a intervenção cirúrgica com identificação do local do trauma, evacuação do hematoma, hemostasia e sutura do corpo cavernoso. Devendo ser associado antibioticoterapia profilática. Casuística: O presente relato tem como objetivo descrever um quadro atípico de fratura de pênis em que o paciente não apresentou “estalido”, nem detumescência como nos casos relatados na literatura. Além disso, a conduta tomada frente ao trauma foi diferenciada, visto que a abordagem cirúrgica ocorreu 18 dias após a fratura. Discussão: O tratamento preconizado, de intervenção cirúrgica imediata foi contraindicada em decorrência do edema volumoso que o paciente apresentava no momento do atendimento inicial. A equipe médica optou por reduzir o edema com o uso de anti-inflamatórios e a abordagem cirúrgica foi realizada tardiamente. Diferentemente do que foi encontrado na literatura, a intervenção cirúrgica é preconizada como tratamento imediato. Conclusão: Concluiu-se que a conduta adotada, reduzir o edema local e abordar cirurgicamente em um segundo momento, foi bem indicada uma vez que o paciente apresentou uma boa evolução.Palavras chave: Fratura, Pênis, Trauma ABSTRACTIntroduction: The penis fracture is an unusual urological emergency and can be defined as closed penile trauma resulting in rupture of the tunica albuginea. The clinical presentation of the patient table is described as a "click" occurred by disruption of the corpus cavernosum, followed by pain, immediate detumescence penile edema and hematoma. During the examination one can see the penile deviation to the opposite side of the fracture. The diagnosis of penile fracture is clinical. The recommended treatment would be surgical intervention with identification of the trauma site, hematoma evacuation, hemostasis and suturing of the corpus cavernosum. It should be associated with prophylactic antibiotics. Case Report: This report aims to describe an atypical case of penile fracture in which the patient did not "click" nor there was detumescence as in the cases reported in the literature. The therapeutic decision facing the trauma was a surgical approach after 18 days of fracture, with clinical treatment in this period. The patient had no complications during the period in which he was assisted. Discussion: Immediate surgery, which is the recommended treatment, was contraindicated in this case due to the massive swelling that the patient showed at the time of initial treatment. The medical team chose to reduce the edema with the use of anti-inflammatory drugs and surgical approach was performed afterwards. Unlikely what was found in the literature, according surgical intervention is recommended as an immediate treatment. Conclusion: This treatment was effective in reducing local edema and made it possible to have the surgical approach in a second moment, being well indicated once the patient showed good evolution.Keywords: Fracture, Penis, Trauma.
