We report the case of an 81 year old lady presenting with a left pulsatile neck mass 7 years after her initial carotid endarterectomy (CEA). We were able to employ a combined hybrid trans-cervical carotid stents followed by open excision and repair for a successful outcome

Paradoxical intravascular bullet embolism involving the aortic arch (AA) is a rare and highly lethal condition. We describe an unusual case of a civilian gunshot injury to the neck. A bullet entered in the neck, injured the internal jugular vein (IJV), and then continued into the lumen of the common carotid artery (CCA). The bullet traveled under its own momentum and against the flow of blood, along the carotid and brachiocephalic vessels, finally lodging in the wall of the lesser curvature of the AA. The injury tract resulted in an arterial-venous fistula between IJV and CCA and a pseudoaneurysm of the AA. Open surgical repair of the neck and AA was complicated by secondary distal embolization of the bullet, requiring an embolectomy.

Pseudoaneurysms of the carotid artery are very uncommon complications following carotid endarterectomy. Pseudoaneurysms are usually caused by any kind of blunt injury or trauma during carotid artery surgery. CEA has become an increasingly more common vascular surgery performed in the United States. The standard of treatment for a carotid PA has been open surgical repair with excision of the defect and then a graft reconstruction of the artery. Advancements in endovascular intervention have helped to make it a more popular choice in treatment because of the positive results and less invasive approach. This case report describes the successful obliteration of a large post-CEA PA using a stent graft. The PA was likely secondary to the use of a Pruitt-lnahara Shunt because it was found to be distal to the endarterectomized area of the carotid artery which means that the defect was likely caused by the balloon portion of the shunt. This case demonstrates the feasibility of using endovascular interventional techniques to treat a PA using a stent graft.

Anastomotic pseudoaneurysm is a known complication following carotid artery endarterectomy and aortobifemoral bypass surgeries. However, giant concurrent carotid and femoral artery anastomotic pseudoaneurysms are rare. We herein report a case of successful treatment of multiple giant anastomotic pseudoaneurysms using a combined endovascular and surgical approach. Our case highlights the fact that endovascular techniques can facilitate open surgical repair of complex anastomotic pseudoaneurysms.
