Objectives:  To describe a case of rapidly relapsing chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) in the setting of positive serum IgG tubulin autoantibodies. Methods: We wrote a case report and performed a literature review of IgG tubulin autoantibodies and the use of rituximab in treatment resistant CIDP. Results: Our case report describes a 29-year-old woman with CIDP that was resistant to treatment with steroids, intravenous immunoglobulin, and plasma exchange. An extensive workup of her rapidly relapsing CIDP was negative, with the exception of positive serum IgG tubulin autoantibodies. She ultimately stabilized on oral steroids, plasma exchange and rituximab, with a regular recurrence of weakness occurring approximately every month that led to rehospitalization. Conclusion: Anti-tubulin antibodies could be a marker of a subtype of CIDP that is treatment resistant. We detail her clinical course to serve as an example for other cases of IgG tubulin autoantibody positive CIDP patients that could be described in the future.

The purpose of the present study was to examine clinical and developmental indices of violent and nonviolent schizophrenic patients. Included in the study were 20 treatment-resistant male schizophrenic patients on fixed doses of neuroleptics; treatment-resistance was determined on the basis of documented failure to respond adequately to treatment with at least three drugs from two different neuroleptic classes. Comparisons of t tests yielded significant differences between the violent and nonviolent patients on the Lion's Violence subscales. Significant differences were also found on the Brief Psychiatric Rating Subscale: violent patients presented more pathologic clinical pictures, were rated higher on physical and motor manifestations of tension, reported more subjective stress, and appeared more hostile and belligerent. Analysis of the WAIS—R data showed that violent patients scored consistently higher on all performance subscales. Violent patients also had fewer prior hospitalizations and a more intact late adolescence. Research is needed to understand better the development and clinical course of violence in schizophrenic patients.

Metastatic renal cell carcinoma (mRCC) has been one of the most treatment-resistant cancers because of its unpredictable clinical course, resistance to chemo- and radiotherapy, and the limited response to immunotherapy and targeted agents. We present a case of long-term survival, that is, 28 years, after primary diagnosis (longest survival in the literature up to our knowledge) with mRCC after several metastasectomies (from local site recurrence, liver, and lung) and eight lines of systemic targeted therapy. This case report shows how crucial is the regular follow-up of patients with RCC after primary management and positive impact of early metastasectomy and systemic targeted therapy in case of mRCC on patients’ condition and overall survival.
