Influence of pain reduction by transcutaneous electrical nerve stimulation (TENS) on somatosensory functions in patients with painful traumatic peripheral partial nerve injury

The pain reducing effect of vibratory stimulation at 100 Hz, transcutaneous electrical nerve stimulation (TENS) at 100 Hz, electroacupuncture at 2 Hz as compared to placebo was studied in 36 patients suffering from myalgia. In 29 patients these modes of stimulation produced pain reduction. Out of these patients 18 rated vibratory stimulation, TENS or electroacupuncture as more effective than placebo while 6 patients rated placebo as more effective. Five patients rated the pain suppressive effect of vibratory stimulation, TENS and electroacupuncture as being equal to that of placebo. Four of the patients who experienced pain reduction with peripheral stimulation reported an effect only for one particular type of peripheral stimulation. The present results provide evidence that vibratory stimulation, TENS and electroacupuncture are as efficient pain suppressive measures and superior to placebo. It is suggested on the basis of these findings that vibratory stimulation, TENS and electroacupuncture merit consideration in the choice of treatment of myalgia.

Introduction. Patients with craniomandibular disorders suffer from hypertonic, fatigued and painful masticatory muscles. This condition can lead to limitation of mandibular jaw movements. All of these symptoms and signs are included in myofascial pain dysfunction syndrome. Transcutaneous electrical nerve stimulation (TENS) has been used for treatment of these patients. Objective. The aim of this study was to assess the effect of TENS therapy on chronic pain reduction in patients with the muscular dysfunction symptom. Methods. In order to evaluate the effect of TENS therapy before and after the treatment, Craniomandibular Index (Helkimo) was used. Pain intensity was measured by VAS. Patients had TENS treatment over two-week period. BURST TENS modality was used. Current intensity was individually adjusted. Results. Two patients did not respond to TENS therapy. Complete pain reduction was recorded in 8 patients, while pain reduction was not significantly different after TENS therapy in 10 patients. Conclusion. TENS therapy was confirmed as therapeutic procedure in orofacial muscle relaxation and pain reduction.
