Premature ovarian failure (POF) is an ovarian defect characterized by the premature depletion of ovarian follicles before the age of 40 years, representing one major cause of female infertility. POF relevance is continuously growing because women tend to conceive ever more frequently in their thirties and forties. POF can present very early with a pubertal defect. More frequently, it is the end stage of an occult process (primary ovarian insufficiency, POI) affecting ∼1–2% of under-40 women. POI is a heterogeneous disease caused by a variety of mechanisms. Though the underlying cause remains unexplained in the majority of cases, various data indicate that POI has a strong genetic component. These data include the existence of several causal genetic defects in humans, experimental and natural models, as well as the frequent familiarity. The variable expressivity of POI defect in women of the same family may indicate that, in addition to some monogenic forms, POI may frequently be considered as a multifactorial defect resulting from the contribution of several predisposing alleles. The X chromosome-linked defects play a major role among the presently known causal defects. Here, we review the principal X-linked and autosomal genes involved in syndromic and nonsyndromic forms of POI with the wish that this list will soon become upgraded because of the discovery of novel contributing mechanisms. A better understanding of POI pathogenesis will indeed allow the construction of tests able to predict the age of menopause in women at higher risk of POI.

Review question / Objective: To assess the effectiveness and safety of Er-xian Decoction in the treatment of primary ovarian insufficiency. Information sources: We will search the following electronic databases, including 3 English databases (PubMed, EMBASE, Cochrane Library) and 4 Chinese databases (China national knowledge infrastructure database, Wanfang database, Sinomed Database, and VIP database). The filters were English and Chinese language. The following key words in Title/Abstract or MeSH search headings are used: “Er-xian” and “Hormone replacement therapy” or “Femoston” or “Climen” and “Primary ovarian insufficiency” or “Ovarian failure” or “Premature ovarian failure” or “POI” and “random*” or “Randomized controlled trial”. In addition, we also search the grey literature such as conference proceedings and dissertations in CNKI and Wanfang database, and relevant trials will be searched in ClinicalTrial.gov database [20] and Chinese Clinical Trial Registry for unpublished trials and protocols. References of all included studies will be hand searched for additional eligible studies.

Background and objective: Mutations in bone morphogenic protein 15 (BMP15) and growth/differentiation factor 9 (GDF9) lead to altered fertility in animal models. In the human, a heterozygous point mutation of BMP15 has been associated with premature ovarian failure (POF). Subject and methods: We have directly sequenced both genes in a cohort of 203 POF patients presenting with primary or secondary amenorrhea and high FSH levels and in a control population including 54 women with regular menstrual cycles who had at least one child. Results: We have identified several heterozygous variants. One alteration in GDF9 (S186Y) and one in BMP15 (L148P) may have pathogenic effects as both positions are conserved in vertebrate species, ranging from the chicken to mammals. These variants were absent in the control samples. We also found synonymous and neutral substitutions. Conclusions: We propose that although mutations in BMP15 and GDF9 are not a major cause of ovarian insufficiency, they may be involved in POF.
