AbstractLiposarcoma in the inguinal region though rare are clinically significant lesions. Preoperative diagnosis is difficult since the clinical findings are very similar to that of inguinal hernia. We report a rare case of Liposarcoma of the spermatic cord in 85 year old male, clinically diagnosed as left sided indirect inguinal hernia. Surgical excision specimen showed multiple globular lipomatous masses which were yellowish and grey tan with areas of myxoid degeneration and necrosis seen. Microscopic examination showed adipocytes arranged in lobules with numerous blood vessels, lipoblasts and myxoid stroma confirming the diagnosis of myxoid liposarcoma. He is on regular follow up since two years without any recurrence or metastasis. Our case report highlights the importance of sampling and examination of fatty masses in the inguinal region to rule out the possibility of liposarcoma as they are mistaken for lipoma at surgery.

Indirect inguinal hernia is a common medical condition that can be caused by an embryological defect due to failure of closure of processus vaginalis that can lead to herniation (i.e. abnormal protrusion) of abdominal contents into scrotal sac in males. In this paper, we describe an interesting case report of a patient with bilateral inguinal hernia with hydroceles who underwent a modified shouldice repair. In addition, we also discuss initial presentation & surgical management of this patient who had painless postero-inferior displacement of testes. Reason why we think this case is unique, is because of ‘bilateral’ hydroceles along with bilateral inguinal hernia which is rarely seen as compared to its unilateral counterparts. We also describe various surgical steps of modified shouldice repair with bilateral orchiopexy along with the post-operative outcome. Finally, we also discuss various types of hydroceles, along with its pathology that is found after surgical resection.South East Asia Journal of Public Health Vol.7(1) 2017: 62-65

Inguinal hernia is relatively uncommon in female, as compared to in males. The ratio of male to female is 6:11. The reason behind that is favorable stronger inguinal anatomy in females. Inguinal hernia in females is quite uncommon compared to males. However, in female it may pose both a diagnostic as well as surgical challenge to the attending surgeon. Awareness of anatomy of the region and all the possible contents is essential to prevent untoward complications. Splenogonadal fusion is a rare congenital malformation, which is an abnormal connection between the spleen and gonads or mesonephric remnants. It usually presents with left cryptorchidism, scrotal mass or left inguinal hernia. Here we present the first case report of splenogonadal fusion as large left scrotal mass leading to abdominoscrotal hydrocele in a 2-year-old boy.
