Background: Junior doctors form the majority of the workforce in patient care. Their job is perilous, highly critical, tedious and exhausting and it is imperative that they stay motivated while at work. Improving the morale of physicians has the potential to increase efficiency, ensure patient safety and improve patient outcomes. We aimed to identify the existing status and explore the factors affecting junior doctors’ morale, their sense of feeling supported and their levels of autonomy in 2 large teaching hospitals in Bangladesh. Methods: This cross-sectional observational study was done across 2 large tertiary hospitals- Bangladesh Institute of Research and Rehabilitation in Diabetes, Endocrine and Metabolic Disorders(BIRDEM) General Hospital and Dhaka Shishu Hospital, over 4 months period from September- December 2020. The study was carried out on 120 junior doctors by an online questionnaire, distributed through emails and Facebook messenger, asking junior doctors to rate their morale, sense of feeling supported and autonomy and rank the top factors that positively affected them. Results: Data were finally collected from 117 junior doctors after 3 incomplete data were discarded. Most of the junior doctors felt ‘neither good nor bad’ in the domains of existing ‘morale’ (44.4%), ‘feeling supported’ (46.5%) and ‘autonomy’ (48.7%). Additionally, ‘good’ morale was seen in 39.3%, while around 34% rated their support system as ‘good’ and around 24% reported a ‘good’ autonomy. The most important factor positively affecting morale was recognition and reward for good performance (70.1%), factor influencing support was an easy access to senior clinicians (70.4%) and that defining autonomy was constant senior supervision of the everyday work (61.1%). Conclusion: The study aims to identify the existing level of morale, support and autonomy of the junior doctors at their workplace and explore the factors positively affecting them. It is concluded from this study that the junior doctors rated their existing morale, support and autonomy as ‘average’. According to the opinions of the doctors, this study also concludes that, to improve their morale, there is a need to recognize and reward their good work and provide positive feedback. Doctors identified an easy access to senior clinicians with a problem was the primary factor influencing support. Finally, junior doctors wanted constant senior supervision of their everyday work in the wards to improve output. BIRDEM Med J 2022; 12(1): 30-35

Background: Guillain-Barre syndrome (GBS) is an acute, frequently severe and fulminant polyradiculoneuropathy that is autoimmune in nature. Incidence and predominant subtypes of GBS differ geographically. Electrophysiology has important role in subtyping GBS. This study aimed to evaluate the electrophysiological findings in patient of GBS. Methods: This was a hospital based cross-sectional descriptive study and conducted at the Department of Neurology in Sir Salimullah Medical College & Mitford Hospital, Dhaka and National Institute of Neurosciences and Hospital, Dhaka during July 2017 to June 2018. Clinically diagnosed 53 patients with GBS were enrolled according to prefixed selection criteria. Detail history taking, clinical examination, nerve conduction study and cerebrospinal fluid (CSF) examination was performed in all cases. Clinical findings, nerve conduction study (NCS) parameters, CSF findings and demographic profiles were evaluated. Results: Mean ± SD age of presentation was 41.64 (±14.56) years and median age was 42.0 years. There were total 33(62 %) males and 20 (38 %) females with male: female ratio of 1.7:1. Clinically two-thirds(62.3%) of patients had both upper and lower limb involvement (62.3%), facial weakness was in 32.1% and 13.2% had bulbar involvement. Acute inflammatory demyelinating polyneuropathy (AIDP), acute motor axonal neuropathy (AMAN) and acute motor sensory axonal neuropathy (AMSAN)were found to be 51%, 32% and 17% respectively. CSFprotein was elevated in most of the patients with a range of 16-725 mg/dl. Highest CSF protein was found in AIDP. Conclusion: Electrophysiological studies play an important role in the early detection; characterization of GBS.In this study, the commonest type of GBS was AIDP. Higher levels of CSF protein, absent H-reflex and Fresponse, sural sparing and unexcitable nerves are more frequently present in AIDP. BIRDEM Med J 2022; 12(1): 16-21

Precocious puberty is commonly defined as puberty that starts before age 8 years in girls and 9 years in boys. The causes of it may range from a variant of normal development to various pathologic conditions. The etiology of precocious puberty is classified by the underlying pathogenesis into gonadotropin dependent central precocious puberty and peripheral precocious puberty which is independent of gonadotropin but due to different other causes. Variants of precocious puberty include premature thelarche, premature puberche and isolated premature menarche which imply onset of isolated changes without any other signs of sexual development. Precocious puberty might have an impact on final stature owing to premature epiphyseal fusion and also it has got influence on psychosocial wellbeing. Evaluation includes a detailed history, physical examination, biochemical testing and imaging directed towards suspected etiology. Gonadotropin releasing hormone (GnRH) analogues are effective for treatment of central precocious puberty. Treatment of peripheral precocious puberty should be based on the specific cause. Pubertal variants are usually non-progressive and need no treatment but should be monitored carefully. BIRDEM Med J 2022; 12(1): 62-69

Background: Medical institutes remained on complete shut down during the coronavirus disease of 2019 (COVID-19) pandemic while Information Technology (IT) bridged the teaching learning between the students and teachers. The study objectives were to determine the opportunities and obstacles of teaching learning process and overall effectiveness of online classes over traditional classes. Methods: A cross-sectional study was conducted from July 2020 to December 2020 between students and teachers of Khulna city Medical College, Khulna, Bangladesh. An online questionnaire was developed using google form containing four sections about different aspects of IT and teaching learning process. Results: Around 87% (160 vs 48) students and teachers responded to the questionnaire. The most preferred online teaching learning platform for students and teachers was zoom (84.4% vs 83.4%) and the favored devices were smart phone (96.2% vs 87.5%) followed by laptop computer (90.6% vs 83.3%). The majority felt connected to each other (82.5% vs 62.5%) and could work faster and effectively (75% vs 66.7%). Conversely, students (65.6%) felt online learning was more enjoyable than teachers (29.2%), where significant difference was found; (p= 0.006). Poor network (98.1% vs 79.1%) and affordability of mobile data (81.2% vs 66.7%) were the two main barriers among them. On-line teaching-learning neither increased student- teacher interaction (51.9% vs 66.7%), nor had better scope of asking question (52.5% vs 70.8%) and there was less scope of explaining details (52.5% vs 66.7%). Oral assessment was the most preferable (89.4% vs 83.3%), but the practical assessment (78.1% vs 83.3%) was the least preferable method for assessing students’ knowledge and skill online. Overall effectiveness of online classes over traditional classes was scored around 50% (±10%) by students and teachers (58.1% vs 62.5%). Conclusion: Despite having barriers like poor network, data affordability and limited computer and net usability, online classes played a pivotal role to continue the academic activities in a medical college during Corona pandemic. Cushing syndrome results from chronic exposure to excess cortisol. Nodular adrenal hyperplasia is usually bilateral and has only a few case reports of unilateral presentation. Biochemically it is presented as hyperaldosteronism or as Cushing’s syndrome. Here, we are reporting a 17-year-old female presenting with weight gain over 5 months and uncontrolled hypertension. Biochemically she was found to have diabetes mellitus, secondary hypothyroidism and hypogonadotrophic hypogonadism due to Cushing’s syndrome of adrenal origin. Unilateral adrenal adenoma/hyperplasia in right adrenal gland was evident by radiology. Histopathological examination was done after laparoscopic adrenalectomy showed nodular adrenocortical hyperplasia in right adrenal mass. Following surgery, clinical features of the patient improved notably. Cushing syndrome due to unilateral nodular adrenocortical hyperplasia is a rare entity. Biochemical evaluation of hypothalamo-pituitary-adrenal axis, radiological evidence and histopathology are the important armaments can guide to final diagnosis. BIRDEM Med J 2022; 12(1): 78-82

Background:The severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has spread throughout the world causing a serious health issue. After the swine flu pandemic (also known as H1N1) of 2009-10, WHO had declared Coronavirus disease 2019 (COVID-19) as a global pandemic. This pandemic caused huge losses to the entire world. Most of the patients with mild symptoms were treated at home but patients with difficulty in breathing and various complications were treated at the hospital. Naturally produced antibodies or vaccination can only offer protection. The objective was to compare the antibody status after SARS-CoV-2 infection in hometreated and hospital-treated patients. Methods: This cross-sectional study was conducted in the Department of Biochemistry and Molecular Biology, BIRDEM General Hospital, from July, 2020 to June, 2021. A total of 77 patients (age 18 – 70 years) infected by SARS-CoV-2 were enrolled for this study. Among them, 37 were home treated and 40 were treated in hospital. The study subjects were non-vaccinated. For this study, serum IgG level was measured by the automated analyser. For statistical analysis, the Mann Whitney U test was done. Results:The median value of serum IgG was significantly higher in hospital-treated patients than in hometreated patients (p < 0.001). Hospital treated patients with a history of comorbidity developed more amount of antibody in comparison to home treated patients. Conclusion:Hospital treated patients develop higher antibodies in comparison to home treated patients. BIRDEM Med J 2022; 12(1): 11-15
