Schizophrenia is a chronic and severe mental disorder characterized by distortions in thinking, perception, emotions, language, sense of self, and behaviour. This report presents the role of clinical pharmacists in the management of a patient diagnosed with schizophrenia with symptoms of paranoia. A gainfully employed young African male adult reported to be roaming around town moving from one bank to another was arrested. The patient was referred to the psychiatric unit of a hospital and diagnosed with schizophrenia. Key interventions offered included rapid tranquilization, electroconvulsive therapy, and psychotherapy. Medications administered to the patient while on admission included IV diazepam, IM haloperidol, IV Ketamine, IM flupentixol, olanzapine tablets, and trihexyphenidyl tablets. Issues raised by clinical pharmacists during the patient’s admission included need for alternative medication for rapid tranquilization, need for initial investigations and documentation of the patient’s vitals, initiation of antipsychotic therapy without initial monitoring and screening for substance abuse, inappropriate dose at initiation of antipsychotic medications, untreated indication, and incidence of missed doses. Interventions by the clinical pharmacists contributed to improvement in the patient’s symptoms prior to hospital discharge. The case proves that it is critical for clinical pharmacists to be involved in the multidisciplinary team during management of patients with psychosis.

The habit of smoking white cigarettes and clove cigarettes may affect the gustatory function, that is, it will cause damage to taste buds, resulting in an increase in gustatory threshold. This research used the descriptive comparative method and had the purpose of obtaining an illustration of gustatory threshold and compare gustatory threshold in white cigarette smokers and clove cigarette smokers in young, male adults. For gustatory threshold evaluation, the Murphy method was used to obtain a value for perception threshold and taste identification threshold using sucrose solution of 0.0006 M-0.06 M concentration. Research results indicate that the perception threshold and identification threshold of young, male adult smokers are 0.0119 M and 0.0292 M. Young, male adult clove cigarette smokers have a perception threshold and identification threshold of 0.0151 M and 0.0348 M. The conclusion of this research is that the perception threshold of young, male adult white cigarette smokers and clove cigarette smokers are the same, whereas the identification threshold of young, male adult white cigarette smokers and clove cigarette smokers are different, that is, the identification threshold of clove cigarette smokers is higher than that of white cigarette smokers.

This paper examines how two manga versions of the Heian classic Tale of Genji, belonging to two different genres and targeting different readership, engage with and interpret the tale’s episodes depicting sexual encounters, which may be read as problematic in the original text. The shojo version, Yamato Waki’s Asaki yumemishi, published between 1980 and 1993, and targeting predominantly female audiences, how two distinct approaches in its treatment of certain potentially uncomfortable episodes: some episodes which verge too close to a reading of sexual violence, are outright erased from the manga versions. Others, whose presence is invaluable to the narrative, are remarkably faithful to the original text, while at the same time contextualizing and domesticating all threats of sexual violence that might have marred the original text. By contrast, Egawa Tatsuya’s seinen version of Genji monogatari, marketed towards a young male adult readership, takes the extreme approach of depicting all sexual encounters in the tale as consensual, pleasurable and highly explicit. The ambiguity of the original text is simply done away with by juxtaposing said text and its fairly accurate rendition into modern Japanese with quasi-pornographic, shunga-evoking scenes of sex.

Abstract Background Simultaneous bilateral femoral neck fracture is a very rare condition, even in osteoporotic elderly individuals. We report an atypical case of a young male adult who developed simultaneous bilateral femoral neck fractures without previous trauma or overuse. Case presentation A 33-year-old man presented with discomfort in the bilateral groin, which had started 2 weeks previously. Bilateral femoral neck fractures were observed on a radiograph, and in addition, a fracture line was seen at the right subchondral region of the acetabulum using magnetic resonance imaging (MRI). Although the patient had no obvious risk factors associated with bone fragility, his bone mineral density measured using dual X-ray absorptiometry indicated severe osteoporosis (lumber spine: T score − 3.4 standard deviation [SD]; femoral neck: T score − 2.8 SD). Serum 25-hydroxyvitamin D level was deficient (19 ng/mL), which was considered to be partly due to non-sunlight exposure for 3 years owing to social withdrawal. Bilateral osteosynthesis was performed, considering his young age, although more than 2 weeks had passed since the onset of the fracture. Bone union and non-occurrence of osteonecrosis of the femoral head were confirmed via radiography and MRI 8 months after the surgery. Conclusions Our case suggests that simultaneous non-traumatic bilateral femoral neck fractures can occur in healthy young men.
