Facial asymmetry, following early childhood condylar trauma, is a common complaint among patients who seek surgical treatment. G.D.M., a 27-year-old male patient, sought professional help to correct his cosmetic flaw, caused by a condylar fracture when he was 8-years-old. After the proper orthodontic treatment, he underwent a double jaw orthognathic surgery and, 9 months later, a second one to correct the remaining asymmetry. Two years after this second procedure, the patient is still under surveillance and has no complaints.

Pendahuluan: Asimetri wajah akibat canting oklusal rahang atas seringkali menjadi keluhan  estetika wajah pasien. Perawatan canting oklusal parah memerlukan kombinasi perawatan ortodonti cekat dengan bedah ortognati. Tujuan laporan kasus ini adalah untuk menjelaskan perawatan ortodonti cekat kombinasi bedah Le Fort 1 pada kasus canting oklusal rahang atas pada maloklusi dentoskeletal kelas III disertai asimetri wajah. Laporan kasus: Seorang pasien perempuan umur 17 tahun 7 bulan datang ke praktek pribadi dengan keluhan gigi rahang atas miring, gigi belakang kanan tidak dapat mengunyah dengan nyaman. Pasien ingin dirawat gigi dan rahangnya. Pemeriksaan ekstra oral menunjukan wajah asimetri, profil cekung dan dagu sedikit menonjol. Pemeriksaan intra oral,  garis median rahang atas bergeser ke kiri, rahang bawah bergeser ke kanan, crossbite anterior, crossbite posterior unilateral, retrusi gigi anterior rahang atas dan rahang bawah. Analisis sefalometri lateral: maloklusi dentoskeletal kelas III. Diagnosis yang diberikan adalah maloklusi dentoskeletal kelas III disertai canting oklusal rahang atas, wajah asimetri, crossbite anterior, crossbite unilateral posterior. Rencana perawatan adalah perawatan ortodonti cekat kombinasi bedah ortognati Le Fort 1. Perawatan dilakukan dalam 4 tahap yaitu perawatan ortodonti dekompensasi, perawatan bedah ortognati rahang atas, perawatan ortodonti pasca bedah rahang, debonding dan pemasangan retainer. Simpulan: Maloklusi skeletal kelas III disertai canting oklusal rahang atas, asimetri wajah, crossbite anterior, dan crossbite posterior unilateral, yang dirawat menggunakan alat ortodonti cekat dan bedah ortognati Le Fort 1 dapat berhasil dengan baik. Relasi dental dan skeletal tercapai kelas I, interdigitasi gigi rahang atas dan rahang bawah mengunci, fungsi pengunyahan terkoreksi serta pasien merasa sangat puas dengan estetika wajahnya.Kata kunci: Maloklusi skeletal kelas III, asimetri wajah, canting maksila, crossbite anterior, crossbite posterior unilateral, bedah ortognati. ABSTRACTIntroduction: Facial asymmetry due to maxillary occlusal cant often becomes a facial aesthetics complaint. Treatment of severe occlusal cant requires a combination of fixed orthodontic treatment with orthognathic surgery. This case report was aimed to describe the combined fixed orthodontic treatment of Le Fort 1 in maxillary occlusal cant of class III dentoskeletal malocclusion with facial asymmetry. Case report: A female patient aged 17 years seven months came to the private clinic, complained of oblique maxillary teeth, and the right posterior was unable to masticate comfortably. The patient wants to be treated for her teeth and jaw. Extraoral examination revealed facial asymmetry, sunken profile and slightly protruding chin. The intraoral examination resulted in the maxillary median line that shifted to the left, mandible shifted to the right, anterior crossbite, unilateral posterior crossbite, and retrusion of maxillary and mandibular anterior teeth. The lateral cephalometric analysis resulted in class III dentoskeletal malocclusion. The diagnosis was class III dentoskeletal malocclusion with maxillary occlusal cant, facial asymmetry, anterior crossbite, and posterior unilateral crossbite. The treatment plan was fixed orthodontic treatment combined with Le Fort orthognathic surgery. The treatment was carried out in 4 stages: decompensated orthodontic treatment, maxillary orthodontic treatment, post-orthognathic surgery orthodontic treatment, debonding, and retainer placement. Conclusion: Class III skeletal malocclusion with maxillary occlusal cant, facial asymmetry, anterior crossbite, and the unilateral posterior crossbite was successfully treated with a fixed orthodontic appliance and Le Fort 1 orthognathic surgery. The dental and skeletal relations were achieved for class I, the interdigitation of the maxillary and mandibular teeth was locked, the masticatory function was corrected, and the patient was very satisfied with her facial aesthetics.Keywords: Class III skeletal malocclusion, facial asymmetry, maxillary cant, anterior crossbite, unilateral posterior crossbite, orthognathic surgery.

Introduction. Orthodontic treatment of Class II Division 1 (II/1) malocclusions in adults can be challenging since skeletal effects are limited. Possible treatment options are orthodontic camouflage or orthognatic surgery, in severe cases. The aim of this paper was to present a successful management of Class II malocclusion in an adult patient using temporary anchorage devices (TADs). Case report. After detailed clinical examination, study models and cephalometric analysis, a 26 years old patient was diagnosed with Class II malocclusion, an overjet of 12 mm, congenitally missing tooth 41 and midline shifted to the right in upper dental arch. In prior orthodontic treatment, patient had upper premolars extracted. Posterior teeth in upper left quadrant were shifted mesially. The camouflage treatment was considered, using temporary anchorage devices (TADs) to distalize posterior teeth on the left side, and gain space for incisor retraction and midline correction in upper dental arch. Results. Using TADs as additional anchorage in anterior region and coil spring for molar distalization, the space was made for tooth 23, midline correction and incisor retraction. After 40 months, a satisfactory result was achieved, overjet and midline correction, class I canines occlusion and class II molar occlusion. Conclusion. Class II/1 malocclusion in adults can be successfully treated using TADs. The success depends on the severity of malocclusion and patient cooperation.

ABSTRACT Introduction: Temporary anchorage devices (TADs) have been developed to be used as direct adjuncts in orthodontic treatment and have facilitated treatment of more complex orthodontic cases, including patients with dental impaction. Objectives: This clinical case reports the applicability of TADs in the orthodontic treatment of a patient with impacted mandibular second molars. Surgical and orthodontic procedures related to the use of miniplates were also discussed in this study. Conclusions: The use of temporary anchorage devices, such as miniplates, can be suggested as an alternative to treat patients with impacted mandibular second molars.

Condylar hyperplasia (CH) is a rare, self-limiting process manifesting between the first and third decades of life. CH causes facial asymmetry and derangement of the occlusion. Management involves resection of the condylar head and orthognathic surgery. This paper describes the case of a 37-year-old woman with spontaneous onset of CH over a span of approximately 25 years. The condition was managed with resection of the condyle alone, which dramatically improved facial asymmetry and altered the occlusion within a few months of follow up. Orthodontic treatment was then carried out and the patient underwent orthognathic surgery after 3 years. The patient is currently satisfied with her appearance and function and there are no signs of recurrence.

As más oclusões do Padrão II podem demandar diferentes abordagens terapêuticas. Quando a face é no mínimo aceitável, o tratamento poderá se restringir a região dentoalveolar. No entanto, quando a face é desagradável a correção deve envolver a realização de cirurgia ortognática, além do tratamento ortodôntico. E é exatamente à união sinérgica de todas as especialidades - Implante, Prótese, Cirurgia e Ortodontia – que garante a construção de um sorriso mais estético e saudável e com melhorias significativas na face. O objetivo desse trabalho é descrever um caso clínico de um paciente adulto, negro, Padrão II, deficiência mandibular grave, face desagradável. O tratamento envolveu ortodontia descompensatória, reabilitação protética, cirurgia ortognática e por fim, reanatomização estética dos dentes anteriores superiores, o que permitiu ao paciente uma condição de normalidade oclusal e facial.Palavras chave: Diagnóstico. When the face is at least acceptable, treatment may be restricted to dentoalveolar region. However, when the is unpleasant, correction should involve performing orthognathic surgery, in addition to the orthodontic treatment. And that is exactly the synergic union of all specialties - Implant, prosthesis, surgery and orthodontics - which ensures the construction of a more aesthetic and healthy smile and with significant face improvements. The aim of this study is to describe a case of an adult patient, black, malocclusions class II, severe mandibular deficiency, unpleasant face. The treatment involved descompensatory orthodontics, prosthetic rehabilitation, orthognathic surgery and finally aesthetic reanatomization the upper front teeth, which allowed the patient a normality condition of occlusion and face.Keywords: Diagnosis. INTRODUCTION: This case report describes the orthodontic treatment of an adult patient with skeletal Class III malocclusion and anterior crossbite. A short cranial base led to difficulties in establishing a cephalometric diagnosis. The patient's main complaint comprised esthetics of his smile and difficulties in mastication. METHODS: The patient did not have the maxillary first premolars and refused orthognathic surgery. Therefore, the treatment chosen was orthodontic camouflage and extraction of mandibular first premolars. For maxillary retraction, the vertical dimension was temporarily increased to avoid obstacles to orthodontic movement. RESULTS: At the end of the treatment, ideal overjet and overbite were achieved. CONCLUSION: Examination eight years after orthodontic treatment revealed adequate clinical stability. This case report was submitted to the Brazilian Board of Orthodontics and Facial Orthopedics (BBO) as part of the requirements to become a BBO diplomate.

A case report is presented which demonstrates the effectiveness of comprehensive orthodontic treatment combined with orthognathic surgery in the correction of malocclusion and reduction in the sequelae of Obstructive Sleep Apnea (OSA). The patient’s severe OSA was improved to very mild as evaluated by full overnight polysomnogram. The orthodontic treatment included the expansion of both dental arches and mandibular advancement surgery. There was significant improvement in the patient’s sleep continuity and architecture with the elimination of obstructive apneas.
