Human bite on genitalia is a rare cause of penile ulceration, but is increasingly being reported, probably due to the increasing frequency of orogenital sex. The great morbidity associated with it brings it under the category of high-risk bite wounds, similar to those on hands, feet and joints. We report a case and review the literature on human bite-induced penile ulceration.

Human bites are an infrequent cause of emergency department visits and hospital admissions. There are rarely published cases of complicated infection, such as infective endocarditis. We present a rare case of a patient with acute infective endocarditis in a healthy native valve and purulent pericarditis from a human bite. A 40-year-old man with obesity suffered deep human bites by an adult woman, with two deep lesions in the anterior thorax and one superficial lesion in the upper abdomen and admitted in intensive care unit with septic shock and a persistent aortic murmur. Echocardiography described evidence of vegetation, perforation and severe regurgitation of aortic valve. Scanner described moderate pericardial effusion. Cardiac surgery was performed, with evidence of purulent pericardial effusion after pericardiotomy, and subsequently aortic valve replacement with a 25 mm bioprosthesis. The patient showed positive progress.

ABSTRACT GAR-936 is a new semisynthetic glycylcycline with a broad antibacterial spectrum, including tetracycline-resistant strains. The in vitro activities of GAR-936, minocycline, doxycycline, tetracycline, moxifloxacin, penicillin G, and erythromycin were determined by agar dilution methods against 268 aerobic and 148 anaerobic strains of bacteria (including Pasteurella, Eikenella,Moraxella, Bergeyella, Neisseria, EF-4, Bacteroides, Prevotella,Porphyromonas, Fusobacterium,Staphylococcus, Streptococcus,Enterococcus, Corynebacterium,Propionibacterium, Peptostreptococcus, andActinomyces) isolated from infected human and animal bite wounds in humans, including strains resistant to commonly used antimicrobials. GAR-936 was very active, with an MIC at which 90% of the strains are inhibited (MIC90) of ≤0.25 μg/ml, against all aerobic gram-positive and -negative strains, including tetracycline-resistant strains of Enterococcus,Streptococcus, and coagulase-negative staphylococci, except for Eikenella corrodens (MIC90, ≤4 μg/ml). GAR-936 was also very active against all anaerobic species, including tetracycline-, doxycycline-, and minocycline-resistant strains ofPrevotella spp., Porphyromonas spp.,Bacteroides tectum, and Peptostreptococcusspp., with an MIC90 of ≤0.25 μg/ml. Erythromycin- and moxifloxacin-resistant fusobacteria were susceptible to GAR-936, with an MIC90 of 0.06 μg/ml.

Different microorganisms not commonly found in the female genital tract have been implicated as causative agents for amniotic fluid infections. Eikenella corrodens is part of the normal flora in the human oral cavity and upper respiratory tract. It is most commonly found in the clinical setting of polymicrobial infections involving human bite wounds, head and neck infections, and periodontitis. On rare occasions, E. corrodens has been reported as a cause of gynecologic infections. There are only 3 reports of documented E. corrodens chorioamnionitis of the placenta resulting in preterm labor, one of which is associated with early neonatal demise. We report a case of preterm delivery of twin infants at 23 weeks of gestation with documented E. corrodens infection in 1 twin who was not resuscitated.

Seventy-three patients with bite wounds (16 patients with clenched-fist injuries, 18 with human bite wounds, and 39 with animal bites) were cultured aerobically and anaerobically. A total of 33 of 34 patients with human bites and clenched-fist injuries and 33 of 39 patients with animal bites had aerobic or facultative bacteria isolated from their wounds. A total of 224 strains of aerobic or facultative bacteria were isolated, the most frequent isolate being alpha-hemolytic streptococci (50 strains). Staphylococcus aureus was isolated from 18 wounds. Penicillin-resistant gram-negative rods were infrequently isolated (12 strains). Anaerobic bacteria were isolated in 18 of 34 human bite wounds and clenched-fist injuries and 16 of 39 animal bite wounds. A total of 88 anaerobic strains was isolated, the most common being various Bacteroides species (36 strains).
